盐酸右美托咪定经鼻术前给药用于小儿麻醉的临床研究 被引量:4

Clinical Study on the Preoperative Transnasal Administration of Dexmedetomidine Hydrochloride in Pediatric Anesthesia

摘要 目的探讨盐酸右美托咪定经鼻术前给药应用于小儿麻醉中的临床价值。方法选取我院2017年10月至2018年6月行外科手术治疗的86例患儿,随机分为对照组和研究组各43例。对照组常规给予咪达唑仑滴鼻进行麻醉诱导,研究组给予盐酸右美托咪定经鼻实施麻醉诱导,比较两组的麻醉诱导效果。结果两组的Ramsay评分、 CHEOPS评分比较有统计学差异(P <0.05)。研究组的入睡时间显著短于对照组(P <0.05)。两组的苏醒时间比较无统计学差异(P>0.05)。研究组的镇静满意度、面罩接受率、静脉穿刺接受率均显著高于对照组(P <0.05)。研究组的术后躁动发生率、呼吸道不良事件发生率均显著低于对照组(P <0.05)。结论盐酸右美托咪定经鼻术前给药应用于小儿麻醉中效果显著,可有效提高镇静、镇痛效果及镇静满意度,减少术后躁动、呼吸道不良事件发生,值得临床推广应用。 Objective To explore the clinical value of the preoperative transnasal administration of dexmedetomidine hydrochloride in pediatric anesthesia. Methods 86 cases of children with surgical treatment from October 2017 to June 2018 in our hospital were selected and randomly divided into control group and study group, with 43 cases in each group. The control group was given midazolam nasal drops for anesthesia induction, and the study group was given the transnasal dexmedetomidine hydrochloride for anesthesia induction. The effect of anesthesia induction was compared between the two groups. Results Statistical difference was found between the two groups in the Ramsay score and CHEOPS score(P<0.05). The sleep time of the study group was significantly shorter than that of the control group(P<0.05). No statistical difference was found in the waking time between the two groups(P >0.05). The sedation satisfaction degree, and the acceptance rates of mask and venous puncture of the study group were significantly higher than those of control group(P<0.05). The incidences of postoperative restlessness and respiratory adverse events of the study group were significantly lower than those of the control group(P<0.05). Conclusions Preoperative transnasal administration of dexmedetomidine hydrochloride in pediatric anesthesia has significant effect,can effectively improve the effects of sedation and anesthesia, and sedation satisfaction degree, and reduce postoperative restlessness and respiratory adverse events, which is worthy of clinical promotion and application.
