摘要
目的探讨IOFIX在第1跖楔关节融合的应用及短期临床效果。方法对10例中重度且伴有跖楔关节松弛的外翻患者行第1跖楔关节融合手术,评价术前及最终随访时第1、2跖骨间夹角(IMA)、外翻角(HAV)、趾AOFAS评分和VAS评分。结果所有患者术后第10周经X线及临床判断均显示第1跖楔关节骨性融合。最终随访时X线片显示融合位置良好,螺钉周围无钉痕。术前HAV平均36°,最终随访时HAV平均13°;术前IMA平均18°,最终随访时平均5°。术前VAS评分平均4.6分,最终随访时VAS评分平均1.2分;术前趾AOFAS评分平均56分,最终随访时趾AOFAS评分平均86分。结论IOFIX用于足部关节融合术的短期临床效果良好,融合率高,同时可避免二次手术取出,但对重度骨质疏松患者慎用。
Objective To explore the short-term clinical effect of IOFIX in the first metatarsowedge joint fusion.Methods 10 patients with hallux valgus with moderate to severe metatarsowedge joint relaxation were treated with first metatarsowedge joint fusion. Preoperative and final follow-up IMA,HAV,AOFAS and VAS scores of the first and second metatarsal angles were evaluated.Results X-ray and clinical judgement showed the first metatarsowedge joint osseous fusion at 10 weeks after operation in all patients.At the end of follow-up,X-ray showed good fusion position and no screw marks around the screw.The average HAV was 36 degree before surgery, reduced to 13 degree after the surgery.The average IMA was 18 degree before surgery,reduced to 5 degree after the surgery.The VAS was 4.6 before surgery,reduced to 1.2 after surgery.AOFAS score of hallux was improved from 56 points to 86 points in average. Conclusion IOFIX has a good short-term clinical effect and high fusion rate in foot arthrodesis,and can avoid secondary surgical removal.However,IOFIX should be used cautiously in patients with severe osteoporosis.
