摘要
探讨妊娠期糖尿病(GDM)患者中肿瘤坏死因子-α(TNF-α)与血管细胞黏附分子-1(VCAM-1)的表达水平并进行相关性研究。方法:随机选取2015年8月~2016年1月在笔者医院分娩的妊娠期糖尿病孕妇50例作为GDM组,正常孕妇50例作为正常组,所有研究对象血浆中TNF-α与VCAM-1浓度的测定采用酶联免疫吸附试验(ELISA),采用免疫组化法测定胎盘组织中TNF-α与VCAM-1的表达,空腹血糖(FBG)、糖化血红蛋白(HbA1c)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A(APOA)、载脂蛋白B(APOB)等生化指标送笔者医院生化检验室检测。结果: GDM组血浆中TNF-α、VCAM-1、FBG、HbA1c、LDL、APOB的水平明显高于正常组(P<0.05),HDL、APOA水平与正常组比较明显降低(P<0.05);GDM组血浆中TNF-α与VCAM-1浓度呈正相关(P<0.05),TNF-α、VCAM-1分别与FBG、HbA1c、LDL、APOB浓度呈正相关(P<0.05),与HDL、APOA浓度呈负相关(P<0.05);正常组血浆中TNF-α与VCAM-1的浓度无相关性(P>0.05),TNF-α、VCAM-1分别与FBG、HbA1c、HDL、LDL、APOA和APOB也没有相关性(P>0.05)。GDM组胎盘滋养细胞中TNF-α和VCAM-1的表达明显高于正常组(P<0.05),且二者呈正相关。此外,血浆中TNF-α水平与胎盘中 TNF-α平均吸光度值呈正相关(P<0.05),血浆中VCAM-1水平与胎盘中VCAM-1平均吸光度值呈正相关(P<0.05)。结论: TNF-α和 VCAM-1与妊娠期糖尿病病情的严重程度相关,并可能通过促进患者的血管内皮细胞损伤在妊娠期糖尿病的发生、发展中起重要作用。
To investigate the expression of tumor necrosis factor-α(TNF-α) and vascular cell adhesion molecule-1 (VCAM-1) in gestational diabetes mellitus patients and conduct a correlation study. Methods Fifty pregnant women with gestational diabetes who were delivered in our hospital from August 2015 to January 2016 were selected randomly as GDM group and 50 normal pregnant women as normal group. The concentrations of TNF-α and VCAM-1 in plasma were determined by enzyme-linked immunosorbent assay (ELISA). The expression of TNF-α and VCAM-1 in placenta was determined by immunohistochemistry. Fasting blood glucose (FBG), hemoglobin A1c (HbA1c), high density lipoprotein (HDL), low density lipoprotein (LDL), apolipoprotein A (APOA)and apolipoprotein B (APOB) were sent to our hospital biochemical laboratory testing. Results The levels of TNF-α, VCAM-1, FBG, HbA1c, LDL and APOB in GDM group plasma were significantly higher than those in normal group ( P <0.05). The levels of HDL and APOA were significantly lower than those in the normal group ( P <0.05). The levels of TNF-α and VCAM-1 in GDM group plasma were positively correlated ( P <0.05). The levels of TNF-α and VCAM-1 respectively were positively correlated with the concentrations of FBG, HbA1c, LDL and APOB ( P <0.05), but negatively correlated with the concentrations of HDL and APOA ( P <0.05). There was no correlation between plasma concentrations of TNF-α and VCAM-1 in the normal group ( P >0.05), and there was no correlation between TNF-α,VCAM-1 and FBG,HbA1c,HDL,LDL,APOA,APOB ( P >0.05). The expression of TNF-α and VCAM- 1 in placental trophoblast cells in GDM group was significantly higher than that in normal group ( P <0.05), and both were positively correlated. In addition, there was a positive correlation between the level of TNF-α in plasma and the average optical density of TNF-α in placenta ( P <0.05). The level of VCAM-1 in plasma was positively correlated with the average optical density of VCAM-1 in placenta ( P <0.05). Conclusion TNF-α and VCAM-1 are associated with the severity of gestational diabetes and may play an important role in the development of gestational diabetes by promoting endothelial cell injury in patients.
