PCR-荧光探针法与RNA恒温扩增法对肺结核诊断的价值 被引量:8

摘要 目的探讨RNA恒温扩增法和PCR-荧光探针法在肺结核临床诊断中的价值及意义。方法以403例临床病原学确诊的肺结核病人为观察组,50例无肺部疾病病人为对照组,分别采用齐-尼痰涂片抗酸染色法、改良罗氏培养基培养法、PCR-荧光探针法和RNA恒温扩增法检测痰标本中结核分枝杆菌,并将检测结果进行对比分析。结果齐-尼痰涂片抗酸染色法、改良罗氏培养基培养法、PCR-荧光探针法和RNA恒温扩增法检测痰标本中结核分枝杆菌的灵敏度分别为37.5%、65.2%、68.7%、60.8%;特异度分别为100%、100%、98%、100%。结论PCR-荧光探针法与RNA恒温扩增法检测结核分枝杆菌快速、敏感、可靠,适合作为肺结核实验室辅助临床诊断的有效检测方法,可为肺结核的早期诊断和早期治疗提供重要依据。 Objective To explore the value and significance of fluorescent probe-based polymerase chain reaction (PCR) and isothermal RNA amplification in the diagnosis of pulmonary tuberculosis. Methods A total of 403 patients with pulmonary tuberculosis as diagnosed by clinical and pathogenic microbiology were enrolled as observation group, and 50 patients without pulmonary diseases were enrolled as control group. All patients were tested for Mycobacterium tuberculosis in the sputum samples using Ziehl-Neelsen stain (an acid-fast bacilli staining method for sputum smears), testing with modified Roche culture medium, fluorescent probe-based PCR, and isothermal RNA amplification, and the test results were compared and analyzed. Results The sensitivity and specificity for detecting Mycobacterium tuberculosis in the sputum samples were 37.5% and 100%, respectively, for Ziehl-Neelsen stain;65.2% and 100%, respectively, for testing with modified Roche culture medium;68.7% and 98%, respectively, for fluorescent probe-based PCR;and 60.8% and 100%, respectively, for isothermal RNA amplification. Conclusion The fluorescent probe-based PCR and isothermal RNA amplification are rapid, sensitive, and reliable in detecting Mycobacterium tuberculosis, which makes them effective laboratory methods for assisting clinical diagnosis;meanwhile, they can provide important clinical evidence for early diagnosis and treatment of pulmonary tuberculosis.
