摘要
目的:探讨临床药师在宫颈癌伴下肢静脉血栓合并阴道出血患者药物治疗中的作用。方法:临床药师参与1例宫颈癌伴下肢静脉血栓合并阴道出血患者的药物治疗过程,根据患者病情,协助医师制订个体化治疗方案。根据止血药的特点,临床药师建议将矛头蝮蛇血凝酶更换为卡络磺钠注射液60～80 mg,临用前加入氯化钠注射液250 ml中静脉滴注,止血治疗,出血较多时联合垂体后叶素注射液,并对患者进行用药宣教;考虑目前患者阴道出血不排除宫颈癌复发所致,建议联合放射治疗。医师采纳了临床药师的建议。结果:患者因止血治疗加重静脉血栓的风险明显降低,出血及静脉血栓同时得到了有效治疗,生活质量得到了提高。结论:临床药师参与患者个体化治疗方案的制订及进行合理用药宣教,可保障患者用药的安全、有效。
OBJECTIVE: To probe into the role of clinical pharmacists in the drug treatment of patient with lower extremity venous thrombosis accompanied with vaginal bleeding. METHODS: The clinical pharmacist participated into the drug treatment process of a patient with lower extremity venous thrombosis accompanied with vaginal bleeding, and assisted the clinicians to formulate individualized treatment plan according to the patient’s condition. According to the characteristics of hemostatic drugs, the clinical pharmacists suggested that the blood coagulase of pikistrodon pikistrodon should be replaced with 60-80 mg of sodium carosulfonate injection, and 250 ml of sodium chloride injection should be added before application for intravenous drip. Hemostatic treatment should be carried out with pituitrin injection when there was a lot of bleeding, and drug education should be carried out for the patient. In consideration of the vaginal bleeding of the patient may be induced by recurrence of cervical cancer, combined radiotherapy was recommended. The clinicians followed the suggestions of the clinical pharmacists. RESULTS: The risk of aggravated venous thrombosis in the patient was significantly reduced by hemostatic treatment, bleeding and venous thrombosis were effectively treated at the same time, and the quality of life was improved. CONCLUSIONS: The participation of clinical pharmacists in the formulation of individualized treatment plan and the promotion and education of rational medication can ensure the safety and effectiveness of medication in patients.
