慢性阻塞性肺疾病患者呼出气一氧化氮与外周血嗜酸性粒细胞相关性分析 被引量:10

Relationship between fractional exhaled nitric oxide and peripheral blood eosinophils in patients with chronic obstructive pulmonary disease

摘要 目的本研究对慢性阻塞性肺疾病(COPD)患者呼出气一氧化氮(FeNO)与外周血嗜酸性粒细胞相关性进行分析。方法回顾性分析本院稳定期COPD患者(稳定期组,n=90)临床资料,同时选取同期门诊正常体检者(对照组,n=100)和急性发作期(急性发作期,n=80) COPD患者为对照。检测患者FeNO、外周血嗜酸性粒细胞水平及肺功能。结果与对照组比较,稳定期组和急性发作期组患者FeNO、外周血嗜酸性粒细胞及诱导痰嗜酸性粒细胞、第1秒用力呼气容积占预计值百分比(FEV1%)、第1秒呼出容积与用力肺活量比值(FEV1/FVC)、C反应蛋白(CRP)、及D-二聚体和白细胞介素-2(IL-2)、白细胞介素-6(IL-6)水平显著升高(P<0.05);稳定期组和急性发作期组FeNO、外周血嗜酸性粒细胞、FEV1%、FEV1/FVC无显著差异(P>0.05),而CRP、D-二聚体、IL-2、IL-6有显著差异(P<0.05);稳定期COPD患者FeNO与血嗜酸性粒细胞呈正相关(r=0.432,P<0.05),与FEV1/FVC呈负相关(r=-0.289,P<0.05)。结论稳定期COPD患者FeNO与血嗜酸性粒细胞呈正相关,而与FEV1/FVC呈负相关。 Objective To explore the relationship of fractional exhaled nitric oxide(FeNO)and peripheral blood eosinophils in patients with chronic obstructive pulmonary disease(COPD).Methods Clinical data of 90 stable COPD patients(COPD stabilized group)were retrospectively analyzed,100 healthy subjects were as control group,and 90 COPD patients in acute period were as acute attack group.FeNO,pulmonary function and peripheral blood eosinophils were detected.Results Compared with the control group,FeNO and peripheral blood eosinophils,induced sputum eosinophil count,percentage of forced expiratory volume in the first second(FEV 1%),ratio of exhaled volume to forced vital capacity in the first second(FEV 1/FVC),C reactive protein(CRP),D-Dimer,interleukin-2(IL-2)and interleukin-6(IL-6)in COPD stabilized group and acute attack group were higher than control group.FeNO,peripheral blood eosinophils,FEV 1%,FEV 1/FVC in COPD stabilized group and acute attack group showed no significant differences(P>0.05),whilst CRP,D-Dimer,interleukin-2,interleukin-6 showed significant differences(P<0.05).There was a positive correlation between FeNO and blood eosinophils in patients with stable COPD(r=0.432,P<0.05),and negatively correlated with FEV 1/FVC(r=-0.289,P<0.05).Conclusion FeNO stable is positively correlated with blood eosinophils and negatively correlated with FEV 1/FVC.
