摘要
目的探究冠心病(CHD)患者血小板参数、血清白介素-6(IL-6)与冠状动脉病变程度的关系。方法选取2015年9月至2018年2月于安徽中医药大学第一附属医院收治的CHD患者204例(CHD组)作为研究对象,其中稳定型心绞痛(SAP)108例,非ST段抬高型急性冠状动脉动脉综合征(NSTE-ACS)76例和ST段抬高型心肌梗死(STEMI)20例,另取68例冠状动脉造影正常者为对照组。按冠状动脉病变Gensini积分将CHD患者分为轻度动脉粥样硬化(AS)组(Gensini<25)98例、中度AS组(25≤Gensini<50)78例和重度AS组(Gensini≥50)28例。测定血小板参数和IL-6水平。结果 CHD组患者血小板计数(PLT)低于对照组(P<0.05),血小板平均体积(WPV)、血小板分布宽度(PDW)、大血小板比率(P-LCR)和IL-6水平高于对照组(P<0.05)。WPV、PDW和IL-6水平在SAP组、STEMI组和NSTE-ACS组依次升高(P<0.05),而PLT在SAP组、STEMI组和NSTE-ACS组依次降低(P<0.05)。随着Gensini积分评分增加,WPV、PDW和IL-6水平依次增加(P<0.05),而PLT水平依次降低(P<0.05)。Pearson相关性分析显示,CHD患者血清IL-6、WPV、PDW与Gensini评分成正相关(r=0.726、0.687、0691,P均<0.05),PLT与Gensini评分成负相关(r=-0.544,P=0.024);而CHD患者IL-6与PLT成负相关(r=-0.372,P=0.039),与WPV、PDW成正相关(r=0.487、0.541、P均<0.05)。结论 CHD患者血小板参数、血清IL-6异常表达,与冠状动脉病变程度密切相关,且血小板参数与IL-6在促进CHD进展中有着协同作用。
Objective To investigate the correlation of platelet parameters and serum interleukin-6(IL-6)levels with severity of coronary lesions in coronary heart disease(CHD).Methods A total of 204 CHD patients admitted in First Affiliated Hospital of Anhui University of traditional Chinese Medicine from September 2015 to February 2018 were selected as CHD group.Among them,108 patients with stable angina(SAP),non-ST segment elevation There were 76 patients with acute coronary artery syndrome(NSTE-ACS)and 20 patients with ST-segment elevation myocardial infarction(STEMI).Another 68 patients with normal coronary angiography were the control group.According to Gensini score,CHD patients were divided into mild atherosclerosis(AS)group(Gensini<25)(n=98),moderate AS group(25≤Gensini<50)(n=78)and severe AS group(Gensini≥50)(n=28).The parameters of platelet and the level of IL-6 was measured.Results Platelet count(PLT)in CHD group was lower than that in control group(P<0.05).The levels of mean platelet volume(MPV),platelet distribution width(PDW),platelet-large cell ratio(P-LCR)and IL-6 in CHD group were higher than those in control group(P<0.05).The level of MPV,PDW and IL-6 increased sequentially in SAP,STEMI and NSTE-ACS group(P<0.05),while PLT level in SAP,STEMI and NSTE-ACS group decreased in turn(P<0.05).With Gensini scores increasing,MPV,PDW,and IL-6 levels increased sequentially(P<0.05),while PLT levels decreased sequentially(P<0.05).Pearson correlation analysis showed that IL-6,MPV,PDW of CHD patients were positively correlated with Gensini score(r=0.726,0.687,0691,P<0.05),and PLT was negatively correlated with Gensini score(r=-0.544,P=0.024).IL-6 of CHD patients was negatively correlated with PLT(r=-0.372,P=0.039),and positively correlated with MPV and PDW(r=0.487,0.541,P<0.05).Conclusion The abnormal expression of platelet parameters and serum IL-6 in CHD patients is closely related to the severity of coronary lesions,and the platelet parameters and IL-6 have a synergistic effect in promoting the progress of CHD.
