摘要
目的:探讨运用团体心理护理干预模式对鼻咽癌放疗期患者进行综合护理干预的效果。方法:采用方便抽样的方法,使用心理痛苦温度计(Dtress Thermometer,DT)测量鼻咽癌放疗患者的心理痛苦水平,分析影响其心理痛苦程度的主要因素。根据心理痛苦程度制定综合护理干预方案,实施团体心理护理干预,出院时调查对护理服务满意度问卷。结果:120例患者,有效完成问卷的患者为116例,心理痛苦得分为(4. 013±2. 293)分,其中DT<4分(轻度痛苦)者占31. 3%,DT 4～6分(中度痛苦)者占43. 1%,DT> 7分(重度痛苦)者占25%。护理人员在患者入院时、放疗开始、放疗中、放疗结束、出院时分别开展团体心理干预护理,出院后患者对护理服务满意率96. 55%,其中不满意占0. 86%、一般占2. 59%、满意占45. 69%、非常满意占50. 86%。结论:重视鼻咽癌住院患者心理痛苦,运用团体心理护理干预模式可以降低患者心理痛苦程度,提高患者依从性,减少负面情绪,积极配合治疗;进一步完善了优质护理服务体系的构建与落实,真正做到以患者为中心,改善医疗服务,增进护患和谐,强化了护士的责任感,提高了护理质量。
Objective:To investigate the status of psychological distress in hospitalized patients with nasopharyngeal carcinoma,and to explore the effect of group counseling in nurse services for nasopharyngeal carcinoma patients with radiotherapy.Methods:Nasopharyngeal carcinoma patients with radiotherapy were selected in convenient and assessed with Dtress Thermometer(DT)to evaluate the status and influencing factors with psychological distress level.Prepared nursing intervention in accordance with the level of psychological distress,implemented the group counseling,and surveyed patients'satisfaction of nursing service with self-compiled questionnaire.Results:Totally 120 patients were selected and 116 patients were collected.In this sample,the scores of DT were 4.013±2.293,and there were 31.3%participants with DT<4(mildly pain),43.1%participants with DT 4-6(moderate pain),25%participants with DT >7(severe pain).Paramedics implemented the group counseling for patients on admission,radiotherapy beginning,after radiotherapy and at discharge respectively.The satisfaction rate of patients to nursing services was 96.55%,and dissatisfied was 0.86%,indifferent was 2.59%,satisfied was 45.69%,quite satisfied was 50.86%.Conclusion:Psychological distress of nasopharyngeal carcinoma patients should be attached more importance,and group counseling could reduce the level of psychological distress of patients,promote the compliancy,reduce negative emotions,actively cooperate with treatment;prefect high quality nursing service system,realize patient-centered,improve health services and nurse-patient harmonious,strengthen nurses'responsibility,and improve nursing services quality.
