摘要
目的分析与研究单纯性肥胖学龄前儿童血清瘦素、胰岛素抵抗、内分泌激素及血脂与生长发育的关系。方法本次研究的对象为2015年6月～2017年12月期间在我院接受治疗的120例单纯性肥胖学龄前儿童以及同时期到我院进行体检的120例正常体重的学龄前儿童,单纯性肥胖学龄前儿童作为观察组,正常体重学龄前儿童作为对照组,比较两组儿童的瘦素、生长激素、皮质醇、甲状腺激素水平,血糖、糖化血红蛋白、胰岛素、胰岛素抵抗指数水平以及血脂水平。结果观察组儿童的瘦素、皮质醇、甲状腺激素水平高于对照组,生长激素水平显著低于对照组,数据差异有统计学意义(P <0.05);观察组儿童的血糖水平低于对照组,差异无统计学意义(P> 0.05),观察组儿童的糖化血红蛋白、胰岛素以及胰岛素抵抗指数水平均高于对照组,差异有统计学意义(P <0.05);观察组儿童的总胆固醇、甘油三脂、低密度脂蛋水平高于对照组,高密度脂蛋白水平低于对照组,差异有统计学意义(P <0.05)。结论单纯性肥胖学龄前儿童存在瘦素抵抗、生长激素分泌减少及内分泌和血脂代谢紊乱等,且严重影响儿童的正常生长发育。
Objective To analyze and study the relationship between serum leptin,insulin resistance,endocrine hormones,blood lipids and growth and development in preschool children with simple obesity.Methods One hundred and twenty preschool children with simple obesity who were treated in our hospital and 120 preschool children with normal weight who underwent physical examination in our hospital from June 2015 to December 2017 were selected as research objects.Preschool children with simple obesity were set as the observation group and preschool children with normal weight were set as the control group.Leptin,growth hormone,cortisol,thyroid hormone level,blood glucose,glycosylated hemoglobin,insulin,insulin resistance index and blood lipid level of children in the two groups were compared.Results The leptin,cortisol,and thyroid hormone level in the observation group were higher than those in the control group,and the growth hormone level was significantly lower than that in the control group,the difference was statistically significant(P<0.05).The blood glucose level in the observation group was lower than that in the control group.There was no statistically significant difference between the two groups(P>0.05).Glycosylated hemoglobin,insulin and insulin resistance index in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).The total cholesterol,triglycerides and LDL levels of the children in the observation group were higher than those in the control group,and the HDL levels were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion Preschool children with simple obesity suffer from leptin resistance,decreased growth hormone secretion,endocrine disorder and lipid metabolism disorder.Simple obesity seriously affects children's normal growth and development.
