摘要
目的探讨开窗引流术与刮治术治疗牙源性颌骨囊性病变的临床效果对比。方法选取我院(2010年1月至2018年1月)收治的100例牙源性颌骨囊性病变患者,均经CT诊断确诊,根据不同治疗方法将本组研究对象分为两组,观察组(n=50)给予开窗引流术治疗,对照组(n=50)给予刮治术治疗,对比两组患者临床疗效和治疗相关指标、术后疼痛发生率、创腔感染发生率、囊腔面积减少率、术后复发率。结果两组患者临床治疗总有效率98.00%/84.00%对比差异明显,具有统计学意义(P<0.05)。观察组患者手术时间、住院时间明显短于对照组,术中出血量明显低于对照组,差异具有统计学意义(P<0.05)。观察组患者术后疼痛和创腔感染发生率6.00%、0.00%明显低于对照组48.00%、14.00%,差异具有统计学意义(P<0.05)。观察组患者术后3个月、6个月囊腔面积率明显大于对照组,差异具有统计学意义(P<0.05)。观察组患者术后术后3个月、6个月复发率0.00%、0.00%明显低于对照组4.00%、8.00%。差异具有统计学意义(P<0.05)。结论开窗减压术治疗牙源性颌骨囊性病变效果优于刮治术,能更有效的促进骨重建,安全性高,值得临床推广。
Objective To compare the clinical effects of fenestration and curettage in the treatment of odontogenic jaw cystic lesions.Methods 100 cases of odontogenic jaw cystic lesions were diagnosed by CT and divided into two groups according to different treatment methods.The observation group(n=50)was treated by fenestration and drainage,and the control group(n=50)was treated by curettage.Bed efficacy and treatment related indicators,postoperative pain incidence,incidence of wound infection,cystic area reduction rate,postoperative recurrence rate.Results The total effective rate of the two groups was 98.00%/84.00%.The difference was significant(P<0.05).The operation time and hospitalization time of the observation group were significantly shorter than those of the control group,and the intraoperative blood loss was significantly lower than that of the control group(P<0.05).The incidence of postoperative pain and wound infection was 6.00%and 0.00%in the observation group,which was significantly lower than 48.00%and 14.00%in the control group(P<0.05).The cystic area rate of the observation group was significantly higher than that of the control group at 3 months and 6 months after operation(P<0.05).The recurrence rate of the observation group was 0.00%and 0.00%at 3 months and 6 months after operation,which was significantly lower than that of the control group(4.00%and 8.00%).The difference was statistically significant(P<0.05).Conclusion The effect of fenestration and decompression is better than curettage in the treatment of odontogenic cystic lesions of the jaw.It can promote bone remodeling more effectively and has high safety.It is worthy of clinical promotion.
