摘要
目的:膝骨关节炎是中老年人常见疾病,西医保守治疗主要以抗炎镇痛,外源性补充关节活性物质为主,口服及外用药物往往具有较大肝肾毒性及消化道刺激性。中药外治法在治疗本病方面有其独特的优势,诸多文献从治疗的中医理论依据及现代医学的作用机理阐释了其药效的作用途径及机理,目前中药外治法主要途径包括传统剂型,如熏、洗、膏敷;新型制剂,如巴布贴、纳米贴,以及结合蜡疗、离子导入等物理疗法。目前中药外治疗法在总结验方进行临床疗效观察及药物机理研究方面取得了一定进展,但对于阐释研究验方药物组成和膝骨关节病(KOA)各辨证分型关系及作用尚有欠缺,因而加强该方向的研究将对进一步提高中药外治法治疗该病的疗效具有很好的临床实际意义。
Knee osteoarthritis is a common disease in middle-aged and elderly people,the conservative treatment of western medicine mainly relies on anti-inflammatory and analgesic,and exogenously supplements joint active substances,oral and topical drugs often have greater hepatorenal toxicity and gastrointestinal irritation.External treatment of traditional Chinese medicine has its unique advantages in the treatment of this disease.Many literatures have explained its pharmacodynamic pathways and mechanisms from the theoretical basis of traditional Chinese medicine and the mechanism of modern medicine,at present,the main external treatment methods of traditional Chinese medicine include:traditional dosage forms such as fumigation,washing,plaster application,new preparations such as cataplasm,nano-patch,and combination of wax therapy,iontophoresis and other physical therapies.At present,some progress has been made in the study of clinical efficacy and drug mechanism of TCM external therapy in the summary of prescriptions,but there is still a lack of clarification and study on the relationship between drug composition and KOA syndrome differentiation,therefore,it is of great clinical significance to strengthen the research in this direction for further improving the curative effect of external treatment with traditional Chinese medicine.
