摘要
目的比较不同手术方法治疗复杂胫骨平台骨折的效果。方法选取我院2016年5月～2017年5月收治的62例复杂胫骨平台骨折患者,根据患者的就诊时间分为观察组(n=31)、对照组(n=31)。观察组采用膝前正中切口双钢板法治疗,对照组采用膝前外侧单切口单钢板法治疗。比较两组的临床效果。结果观察组的术中出血量明显少于对照组,骨折愈合时间明显短于对照组,内固定失败率低于对照组,膝关节功能优良率高于对照组,差异有统计学意义(P<0.05)。其中,骨折后6～8 d实行手术治疗为最佳手术时机,能有效控制术后并发症发生率。结论复杂胫骨平台骨折的手术时机建议在骨折后6～8 d,可将膝前正中切口双钢板法治疗作为首选。
Objective To compare the effect of different operative methods in treatment of complex tibial plateau fracture.Methods From May2016to May2017,62patients with complex tibial plateau fracture treated in our hospital were selected and they were divided into the observation group(n=31)and control group(n=31)according to the visit time of patients.The observation group was treated with double plate method of knee anterior median incision,and the control group was treated with single plate method of knee anterior lateral incision.The clinical effect between the two groups was compared.Results The bleeding amount during operation was significantly less than that the control group,and fracture healing time was significantly shorter than that the control group,internal fixation failure rate is lower than that in the control group,and the good and excellent of knee joint function was higher than that of control group in the observation group,and the differences were statistically significant(P<0.05).Among them,surgical treatment was carried out6-8days after fracture,which was the best time for surgery and could effectively control the incidence of postoperative complications.Conclusion Operative timing for complex tibial plateau fractures is suggested to be6-8days after fracture,and the double plate method of knee anterior median incision should be the first choice.
