摘要
目的:比较不同吻合方式在腹腔镜辅助右半结肠切除术中的疗效。方法:回顾分析3年来为193例结肠癌患者行腹腔镜辅助右半结肠切除术的临床资料,按吻合方式将患者分为观察组(n=99)与对照组(n=94),观察组采用回结肠侧侧吻合,对照组采用端侧吻合,比较分析两组患者临床资料并随访术后生存期。结果:两组手术时间、术后首次排气时间、排便时间、12 d排便次数、术中出血量差异均无统计学意义(P>0.05);观察组吻合口漏、吻合口狭窄、吻合口出血及肠梗阻等并发症总发生率低于对照组,差异有统计学意义(P<0.05),两组患者术后生存期差异无统计学意义。结论:腹腔镜辅助右半结肠切除术中采用侧侧吻合术后并发症发生率低于端侧吻合,侧侧吻合术可作为优选方式。
Objective:To compare the effect of different anastomosis methods in laparoscopic assisted right hemicolectomy.Methods:In the last 3 years 193 patients who suffered from colon cancer and underwent laparoscopic assisted right hemicolectomy were randomly divided into the observation group(n=99)and the control group(n=94)according to the anastomosis method.The side-to-side ileocolon anastomosis was performed in the observation group and the end-to-side anastomosis was used in the control group.The clinical data of the two groups were analyzed and compared,and the postoperative survival period was followed up.Results:There was no significant difference between the two groups in the operative time,postoperative first exhaust time,defecation time,frequency of 12-days-defecation and intraoperative amount of bleeding(P>0.05).The total incidence of complications including anastomotic leakage,anastomotic stenosis,anastomotic bleeding and intestinal obstruction in the observation group was significantly lower than that in the control group(P<0.05).And there were not any differences in survival period between the 2 groups.Conclusions:In laparoscopic-assisted right hemicolectomy,side-to-side anastomosis is associated with a lower incidence of complications compared with the end-to-side anastomosis.It is suggested that side-to-side anastomosis should be chosen optimally in laparoscopic-assisted right hemicolectomy.
