摘要
目的比较临床常用新生儿巨细胞病毒的检测方法。方法收集193例新生儿的血液与尿液标本,分别采用ELISA法检测抗体CMV-IgM和实时荧光定量PCR法检测血清和尿液CMV-DNA,并对检测结果进行比较分析。结果 193例患儿中,血清抗体CMV-IgM阳性51例,检出率为26.4%;尿液HCMV-DNA阳性92例,检出率为47.7%;两种检测方法比较,差异有统计学意义(P<0.05),一致性强度指标Kappa=0.46,吻合度一般。血清HCMVDNA阳性14例,检出率为7.3%,与尿液检出率比较,差异有统计学意义(P<0.05),一致性强度指标Kappa=0.158,两种检测方法吻合度较差。结论尿液CMV-DNA较血清抗体CMV-IgM和血液CMV-DNA检出率高,可与IgM联合检测提高诊断准确性,减少漏诊的发生。
Objective To compare the methods for detection of neonatal cytomegalovirus in clinical practice.Methods The blood and urine samples of 193 newborns were collected.The antibody CMV-IgM was detected by ELISA method and the serum and urine CMV-DNA were detected by real-time fluorescence quantitative PCR,and the results were compared and analyzed.Results Of the 193 children,51 cases were positive for serum antibody CMV-IgM,and the positive rate was 26.4%.Urine HCMV-DNA was positive in 92 cases,the positive rate was 47.7%.There were significant differences between the two methods(P<0.05).The consistency intensity index(Kappa)was 0.46,and the coincidence degree is general.Positive rate of serum HCMV-DNA in 14 cases(7.3%)was significantly different from that in urine(P<0.05).The Kappa value of consistent strength index was 0.158,and the coincidence between the two methods was poor.Conclusion The detection rate of urine CMV-DNA is higher than that of serum antibody CMV-IgM and blood CMV-DNA.It could be combined with IgM to improve the diagnostic accuracy and reduce missed diagnosis.
