Fraxion放射治疗定位系统在颅内肿瘤立体定向放射治疗中的临床应用探讨 被引量:6

摘要 目的:探讨Fraxion放射治疗定位系统的临床应用价值。方法:采用Fraxion放射治疗定位系统对2例颅内肿瘤患者进行体位固定,完成放射治疗计划后在加速器下行位置验证,实施立体定向放射治疗,启动加速器机载锥形束CT(CBCT)程序进行扫描获取治疗部位三维影像,与计划CT影像进行比对。结果:应用Fraxion放射治疗体位固定系统进行颅内的立体定向放射治疗体位固定,患者体位舒适,自动识别和抽吸口腔内的唾液;锥形束CT(CBCT)验证提示,X轴即患者的左右方向线性误差最大值为0.1 cm,均值为(0.08±0.04)cm,旋转角度最大值为0.8°,均值为(0.53±0.30)°,Y轴方向上即患者的头脚方向线性误差最大值为0.3 cm,均值为(0.20±0.06)cm,旋转角度最大值为1.5°,均值为(0.88±0.58)°,Z轴方向即患者的前后方向线性误差最大值为0.1 cm,均值为(0.01±0.04)cm,旋转角度最大值为1.5°,均值为(1.08±0.44)°,三维影像与计划CT影像进行比较,其差异有统计学意义(t=7.00,t=4.92,t=10.18,t=4.25,t=1.90,t=6.91;P<0.05)。结论:应用Fraxion放射治疗体位固定系统进行头部颅内的放射治疗体位固定,能够满足放射治疗靶区的治疗精度和重复性,并可自动识别和抽吸口腔内唾液,保证了治疗安全。 Objective:To investigate the clinical application of the Fraxion localization system in the stereotactic radiotherapy(SRT).Methods:The Fraxion localization system for radiotherapy was used to fix the head position of two patients with intracranial tumors.After the radiotherapy plan was completed,they received position verification under accelerator,and the stereotactic radiotherapy was implemented.And then,CBCT progress that carried on accelerator was started to scan and obtain 3D images about treated position.Finally,the results were compared with planning CT images.Results:The patients with encephalic tumors were comfortable while the Fraxion radiotherapy system for fixed position was used.The oral saliva could be automatically recognized and pumped out.The results of the CBCT showed that the mean of linear error of X-axis(that meant the left-right direction)were 0.08±0.04cm and its biggest was 0.1cm,and the biggest value and mean value of rotation angle was 0.8°and(0.53±0.30)°,respectively.On the Y-axis direction(that meant the head-foot direction of patients),the biggest value and mean of linear error were 0.3cm and(0.20±0.06)cm,and the biggest value and mean of rotation angle were 1.5°and(0.88±0.58)°,respectively.In addition,the biggest value and mean of linear error of Z-axis(that meant the front-back direction of patients)were 0.1cm and(0.01±0.04)cm,respectively,and its biggest value and mean of rotation angle were 1.5°and(1.08±0.44)°,respectively.The difference between 3D images and planned CT images was significant(t=7.00,t=4.92,t=10.18,t=4.25,t=1.90,t=6.91,P<0.05).Conclusion:The application of Fraxion radiotherapy system for fixed position of patients with encephalic tumor can satisfy curative precision and repeatability of target area of radiotherapy,and can automatically identify and pump out oral saliva.Therefore,it can guarantee therapy safety.
