摘要
目的探讨血清纤维蛋白原/白蛋白(FAR)与非ST段抬高型急性冠脉综合征(NSTE-ACS)患者冠状动脉病变程度的关系。方法选择NSTE-ACS患者216例,根据SYNTAX评分标准分为冠脉轻度病变组98例、中度病变组73例、重度病变组45例。入院后检测血清纤维蛋白原、白蛋白水平,计算纤维蛋白原/白蛋白比值(FAR)。采用Spearman相关性分析FAR与NSTE-ACS冠脉病变程度的关系,Logistic回归分析NSTE-ACS患者发生冠状动脉中重度病变(SYNTAX评分≥23分)的危险因素,受试者工作特征曲线评价FAR诊断NSTE-ACS患者发生冠状动脉中重度病变的价值。结果轻度病变组、中度病变组、重度病变组FAR比较有统计学差异(P<0.05),且随着SYNTAX评分升高,FAR逐渐升高(P均<0.05)。FAR与SYNTAX评分呈正相关(r=0.550,P<0.05),FAR升高是患者冠状动脉中重度病变的独立危险因素(P<0.05);FAR界值为118.58诊断NSTE-ACS患者中SYNTAX评分≥23分的曲线下面积(AUC)为0.856(95%CI:0.802~0.900),敏感性为59.32%、特异性为93.88%;FAR界值为126.77诊断NSTE-ACS患者SYNTAX评分≥23分的AUC为0.906(95%CI:0.859~0.941),敏感性为82.22%、特异性为89.47%。结论 NSTE-ACS患者FAR水平与冠脉病变程度呈正相关,FAR水平对冠状动脉中重度病变具有一定预测价值。
Objective To investigate the relationship between the fibrinogen/albumin ratio(FAR)and the degree of coronary artery disease in patients with non-ST-elevation acute coronary syndrome(NSTE-ACS).Methods A total of 216 patients with NSTE-ACS were selected.According to the SYNTAX score,patients were divided into 98 cases with mild disease(mild group),73 cases with moderate disease(moderate group),and 45 cases with severe disease(severe group).After admission,the levels of fibrinogen and albumin were measured and the serum fibrinogen/albumin ratio(FAR)was calculated.Spearman correlation analysis was used to analyze the relationship between FAR and NSTE-ACS coronary artery lesions.Logistic regression analysis was used to analyze the risk factors for moderate to severe coronary artery disease(SYNTAX score≥23 points)in NSTE-ACS patients.The receiver operating characteristics(ROC)curve were used to evaluate the value of FAR in the diagnosis of severe coronary artery disease in NSTE-ACS patients.Results There were significant differences in FAR levels between mild,moderate,and severe groups(P<0.05).As the SYNTAX score increased,FAR increased gradually(P<0.05).Spearman correlation analysis showed that there was a positive correlation between FAR and SYNTAX scores(r=0.550,P<0.05).Multivariate logistic regression analysis showed that FAR was an independent risk factor for severe coronary artery disease(P<0.05).The area under the ROC curve(AUC)of FAR with a cutoff of 118.58 for the diagnosis of NSTE-ACS patients with a SYNTAX score of≥23 was 0.856(95%CI:0.802-0.900),with the sensitivity of 59.32%,and the specificity of 93.88%;AUC of FAR with a cutoff of 126.77 for the diagnosis of NSTE-ACS patients with a SYNTAX score of≥23 was 0.906(95%CI:0.859-0.941),with the sensitivity of 82.22%,and the specificity of 89.47%.Conclusion The level of FAR in NSTE-ACS patients is positively correlated with the severity of coronary artery disease.FAR has certain predictive value for moderate to severe coronary artery disease.
