应用峰值应变离散度评价原发性高血压患者左心室纵向收缩同步性 被引量:9

Assessment of Left Ventricular Longitudinal Systolic Synchrony in Patients with Essential Hypertension by Peak Strain Dispersion

摘要 目的探讨纵向应变达峰时间离散度评价左心室射血分数保留原发性高血压患者左心室纵向收缩同步性的应用价值。资料与方法选择左心室射血分数(LVEF)>52%的50例原发性高血压病患者为高血压组,并进一步分为无左心室肥厚(NLVH)组(n=21)和左心室肥厚(LVH)组(n=29);选取同期健康体检者26例为对照组。所有研究对象均测量常规超声心动图参数,计算左心室质量指数(LVMI)、LVEF。应用Echopac201.54软件获得18节段纵应变达峰时间(TTPLS)牛眼图及纵向峰值应变离散度(PSD),并计算各组的左心室纵向应变达峰时间极差值(TTPLSR)和左心室不同步节段个数(LVDSN)。结果对照组、NLVH组和LVH组的PSD、TTPLSR、LVDSN逐步增加,差异均有统计学意义(P<0.05)。高血压组患者PSD、TTPLSR、LVDSN均与LVMI呈正相关(r=0.75、r=0.48、r=0.43,P<0.05)。PSD、TTPLSR、LVDSN的受试者工作特性曲线下面积依次为0.90、0.86、0.81,由阈值获得三者的敏感度、特异度分别为76%、92.3%,74%、80%,92%、58%。结论应用PSD能够较好地评价LVEF保留高血压患者左心室的纵向收缩同步性。 Purpose To investigate the value of peak strain dispersion in evaluation of left ventricular longitudinal systolic synchrony in essential hypertension patients with preserved ejection fraction.Materials and Methods Fifty essential hypertension patients with preserved ejection fraction in hypertension group were further assigned into two groups,left ventricular hypertrophy(LVH)group(n=29)and non-left ventricular hypertrophy(NLVH)group(n=21);26 healthy volunteers were enrolled as control group.All the subjects were measured for conventional echocardiographic parameters,left ventricular mass index(LVMI)and left ventricular ejection fraction(LVEF)were calculated.The bull’s-eye plot of the corresponding 18-segmental time to peak longitudinal strain(TTPLS)and peak strain dispersion(PSD)were obtained using Echopac 201.54 software.In addition,the time to peak longitudinal strain range(TTPLSR)and left ventricle dyssynchrony segments number(LVDSN)were calculated.Results PSD,TTPLSR and LVDSN increased gradually in control group,NLVH group and LVH group,with statistically significant differences(P<0.05).PSD,TTPLSR and LVDSN were all positively correlated with LVMI in hypertension group(r=0.75,r=0.48 and r=0.43,respectively,P<0.05).The area under the curve of PSD,TTPLSR and LVDSN in diagnosing synchrony were 0.90,0.86 and 0.81,respectively.The sensitivity and specificity of PSD were 76%and 92.3%,of TTPLSR were 74%and 80%,and of LVDSN were 92%and 58%,obtained by cutoff value.Conclusion PSD can be used to evaluate left ventricular longitudinal strain synchrony in essential hypertension patients with preserved ejection fraction.
