摘要
目的探讨不同类型肺癌在自发荧光气管镜(AFB)与白光气管镜(WLB)下特点以及纤维支气管镜刷片(BWC)与支气管肺泡灌洗术(BALF)在肺癌诊断中的价值。方法对641例经AFB联合WLB检查首次确诊为肺癌的气管镜检查结果进行回顾性分析。结果 641例肺癌中鳞癌267例、腺癌179例、小细胞癌150例、其他45例,WLB下以增生性病变为主,炎症性病变最少,WLB检出肺癌513例(80.0%),AFB检出肺癌576例(89.9%),增生、浸润、狭窄、炎症性病变组AFB诊断肺癌敏感度均高于WLB,差异具有统计学意义。鳞癌、腺癌、小细胞癌BWC联合BALF阳性率均高于BWC或BALF阳性率,差异具有统计学意义。结论支气管肺癌在气管镜下表现具有一定规律,临床疑诊支气管肺癌者应常规行AFB联合WLB检查,单独BWC或BALF诊断肺癌阳性率较低,联合检测可增加肺癌检出率。
Objective To investigate the characteristics of different types of lung cancer under autofluores?cent bronchoscopy(AFB)and white light bronchoscopy(WLB)and the value of bronchofiberscopic washing cytology(BWC)and bronchoalveolar lavage(BALF)in the diagnosis of lung cancer.Methods The bronchoscopy results of 641 patients diagnosed as lung cancer by AFB combined with WLB were analyzed retrospectively.Results In 641 cases,there were 267 cases of squamous cell carcinoma,179 of adenocarcinoma,150 of small cell carcinoma and 45 of others.The sensitivity of AFB in diagnosing lung cancer was significant higher than that of WLB in proliferating,infiltrating,stenosis and inflammatory lesions.The positive rate of squamous cell carcino?ma,adenocarcinoma,and small cell carcinoma detected by BWC combined with BALF was significant higher than that by BWC or BALF alone.Conclusions Characteristics of lung cancer under the bronchoscope have a certain rule,thus,patients with clinical suspicion of bronchial lung cancer should have routine AFB combined with WLB.Positive rate of lung cancer detected by BWC or BALF is low but combined detection can increase the detection rate of lung cancer.
