摘要
目的:探讨不同疗程布拉氏酵母菌散联合四联疗法根除幽门螺旋杆菌的效果。方法:选取210例消化性溃疡伴幽门螺旋杆菌阳性患者,随机分为A组、B组和C组各70例,A组给予四联疗法,雷贝拉唑片0.02g/次,2次/d,枸橼酸铋钾胶囊0.2g/次,2次/d,呋喃唑酮片0.1g/次,2次/d,阿莫西林胶囊1.0g/次,2次/d,疗程为2周。B、C组在A组基础上分别加服2周和4周布拉氏酵母菌散250mg/次,2次/d。完成治疗后4周检测幽门螺旋杆菌清除情况,并评估不良反应情况。结果:A组有7例,B组有3例,C组有2例脱落。A、B、C组Hp根除率分别为80.95%(51/63),95.52%(64/67),97.06%(66/68),差异有统计学意义(P<0.05)。两两比较结果显示:B组、C组较A组Hp根除率明显提高(P<0.0167);B组与C组间根除率差异无统计学意义(P<0.0167)。A、B、C组治疗不良反应发生率分别为14.29%、13.43%和10.29%,差异无统计学意义(P>0.05)。结论:2周或4周布拉氏酵母菌散联合四联疗法均可以提升Hp根治率,不增加不良反应,建议推广应用2周布拉氏酵母菌散联合四联疗法。
[Objective]To explore whether Saccharomyces boulardii sachets can improve the effects when adding in the standard helicobacter pylori quadruple therapy.[Method]210 digestive ulcer patients with positive h.pylori were selected and randomly divided into Group A(70 cases),Group B(70 cases)and Group C(70 cases)Group A were treated with quadruple therapy:Rabeprazole pill,20mg,bid,Bismuth Potassium Citrate Capsule,0.2g,bid,Furazolidone Tablet,0.1g,bid,Amoxicillin capsule,1g,bid.Group B and C were treated with brady's yeast powder(250mg,bid,po)2 weeks or 4 weeks on the baseline treatment of Group A.After 4 weeks of treatment,helicobacter pylori detection was made to evaluate clearance and side effects were surveyed.[Result]There were 7,3 and 2 cases drop out in Group A,B,C in the research.Hp eradication rate were 8 0.95%(51/63),95.52%(64/67),97.06%(66/68)respectively,with a marked statistical difference(P<0.0 5),in which the eradication rates in Group B and C were higher than Group A(P<0.0167)and there was no statistical significance in eradication rate between Group B and Group C(P<0.0167)The side effects rates were 14.2 9%,13.4 3%,10.29%in Group A,B,C respectively,without marked difference(P>0.0 5)[Conclusion]Compared with the standard quadruple therapy,adding brady's yeast powder 2 or 4 weeks both can significantly improve the eradication rate of Hp and keep the same level of side effects rate as standard therapy.We suggest popularizing 2 weeks Saccharomyces boulardii sachets combined with quadruple therapy.
