新活素对老年失代偿性心力衰竭患者心功能的影响 被引量:12

Influence of recombinant human brain natriuretic peptide on heart function in elderly patients with decompensated heart failure

摘要 目的探讨新活素对老年失代偿性心力衰竭患者心功能影响。方法选择中国人民解放军464医院干部病房科于2013年3月~2017年5月间住院的老年失代偿性心力衰竭患者82例作为研究对象,按照入院顺序分为观察组和对照组,每组各41例,两组入选患者均给予常规抗心力衰竭治疗,在此基础上对照组予以硝酸甘油治疗,观察组予以新活素治疗,观察治疗调整前后对比两组患者的心功能、N端-B型脑利钠肽前体(NT-pro BNP)及临床疗效。结果治疗前两组患者的左室射血分数(LVEF)、左心室舒张末内径(LVDd)及血浆NT-pro BNP比较,差异无统计学意义(P>0.05);治疗后观察组患者与治疗前比较LVEF、LVDd及NT-pro BNP水平均明显改善,与对照组比较,观察组LVEF、LVDd及NT-pro BNP水平改善更为明显,P<0.05,差异均具有统计学意义。观察组临床总有效率为92.11%,明显高于对照组的73.17%,P<0.05,差异均有统计学意义。结论新活素对老年失代偿性心力衰竭患者临床疗效显著,可明显改善心功能水平及心室重塑。 Objective To investigate the influence of recombinant human brain natriuretic peptide(rhBNP)on heart function in elderly patients with decompensated heart failure(DHF).Methods Elderly DHF patients(n=82)were chosen from the Department of Geriatrics of Chinese PLA 464 Hospital from Mar.2013 to May 2017,and divided,according to order of hospitalization,into observation group and control group(each n=41).All patients in 2 groups were treated with routine anti-heart failure therapy,and control group was additionally given nitroglycerin(NTG)and observation group,additionally,rhBNP.The heart function,level of N-terminal pro-brain natriuretic peptide(NT-proBNP)and clinical efficacy were observed and compared in 2 groups.Results The comparison in left ventricular ejection fraction(LVEF),left ventricular end-diastolic inner diameter(LVEDd)and NT-proBNP showed that difference had no statistical significance(P>0.05).LVEF,LVEDd and NT-proBNP were significantly improved in observation group after treatment,which was more significant in observation group than that in control group(P<0.05).The total clinical efficacy was 92.11%in observation group and 73.17%in control group(P<0.05).Conclusion RhBNP has significant clinical efficacy,and can improve heart function and relieve ventricular remodeling in elderly DHF patients.
