摘要
目的观察在无静脉可供吻合手术末节断指再植中应用单侧指动脉结扎的效果,探讨缺乏可吻合静脉的末节断指的治疗方法。方法选择在浙江省乐清市第三人民医院接受治疗的56例(62指)末节断指患者进行研究,将其随机分为对照组和观察组,每组28例。对照组给予吻合指动脉断指再植术治疗,观察组患者同时给予单侧指动脉结扎治疗。比较两组患者的治疗效果。结果观察组患者再植成活率明显高于对照组,指体坏死和血管危象发生率明显低于对照组(P<0.05);两组患者手术时间比较差异无统计学意义(P>0.05),而观察组患者的创口愈合时间及住院时间均明显短于对照组(P<0.05);术后对患者进行随访,观察组患者术后再植指体的指甲长度、感觉恢复、静态两点辨别觉(2-PD)及指关节活动度(DIPJ)活动度均明显优于对照组(P<0.05)。结论单侧指动脉结扎治疗缺乏可吻合静脉的末节断指再植效果显著,可有效提供再植指体的成活率,同时降低术后静脉危象等并发症的发生,治疗后患者指体外观及功能恢复良好,值得推广应用。
Objective To observe the effect of unilateral refers artery ligation without venous replantation in the application of finger replantation and to discuss the intravenous therapy.Methods 56 cases(62 fingers)treated in Yueqing city,zhejiang province,the third people's hospital were randomly divided into control group and observation group,28 cases in each group.The control group was given anastomosis wounded finger artery treatment,and the observation group received unilateral artery ligation treatment.Compare the therapeutic effects of two groups.Results Survival rate of replantation in observation group was obviously higher than that in control group.Incidence of necrosis and vascular crisis in observation group were significantly lower than those in control group(P<0.05).Operation time had no significant difference in both groups.Wound healing time and hospital stay in the observation group were significantly shorter than those in the control group(P<0.05).The nail length of body,feeling returned,2-PD and DAPJ activity in observation group were significantly better than those in control group(P<0.05).Conclusion Unilateral artery ligation anastomosis of vein is effective and can effectively increase the survival rate of replantation.Body appearance and functional recovery after treatment are good.It can also reduce the occurrence of complications such as postoperative venous crisis and is worth popularization and application.
