TCT、HPV-DNA分型联合HPVL1壳蛋白检测宫颈癌的临床价值 被引量:4

Clinical value of TCT, HPV-DNA combined with HPVL1 capsid protein in the detection of cervical cancer

摘要 目的研究液基薄层细胞学(TCT)、高危型人乳头瘤病毒(HPV-DNA)分型联合人乳头瘤病毒L1(HPVL1)壳蛋白检测宫颈癌的临床价值。方法回顾性分析我院收治的170例宫颈病变患者的临床资料及宫颈脱落细胞标本,以病理学结果为准,所有患者均经TCT、HPV-DNA分型联合HPVL1壳蛋白检测,分析三种方法单独使用及联合使用对宫颈癌、癌前病变筛查的灵敏度、特异度、阳性预测值及阴性预测值。结果 HPV-DNA分型阳性诊断符合率高于HPVL1壳蛋白和TCT检测,HPVL1壳蛋白阳性诊断符合率高于TCT检测(P<0.05)。HPV-DNA分型检测阳性符合率与病理结果最接近,达95.29%。HPVL1壳蛋白检测诊断的阳性符合率随着宫颈病变等级的升高而逐渐下降。三者联合诊断CIN2以上病变的灵敏度、特异度、阳性预测值、阴性预测值均较单独应用时有所提高。结论 TCT、HPV-DNA分型检测是宫颈癌筛查中的重要指标,而HPVL1壳蛋白检测对宫颈病变进展有一定预测价值,可避免过度诊断,三者联合检测在宫颈癌诊断的精准性方面有重要意义。 Objective To investigate the clinical value of TCT,HPV-DNA combined with HPVL1 capsid protein in the detection of cervical cancer.Methods The clinical data and cervical exfoliated cell specimens of 170 patients with cervical lesions in our hospital were retrospectively analyzed.The gold standard was based on the pathological results.All the patients were detected by TCT,HPV-DNA and HPVL1 capsid protein.The sensitivity,specificity,positive predictive value and negative predictive value of solo and combined detection with these three methods were analyzed.Results The positive coincidence rate of HPV-DNA was higher than that of the HPVL1 capsid protein and TCT detection,and the positive coincidence rate of HPVL1 capsid protein was higher than that of the TCT detection(P<0.05).The positive coincidence rate of HPV-DNA with pathological results were the closest,reaching 95.29%.The positive coincidence rate of HPVL1 capsid protein detection gradually decreased with the increase of cervical lesion grade.The sensitivity,specificity,positive predictive value and negative predictive value of the three combined detection of CIN2 and above lesions were all higher than those of solo detection.Conclusion The detection of TCT and HPV-DNA are important indicators of cervical cancer screening,and the detection of HPVL1 capsid protein has some predictive value for the development of cervical lesions,these three joint detection is of great significance in the diagnosis of cervical cancer.
