摘要
目的探讨锁定钢板植骨内固定配合活血补肾壮骨汤治疗胫骨中下段骨折不愈合的疗效。方法选取河南省洛阳正骨医院2015年2月至2017年5月住院治疗的胫骨中下段骨折术后不愈合的患者86例,随机分为观察组和对照组,每组43例。两组全部手术采用锁定钢板并取自体髂骨椎骨内固定,观察组在此基础上联合服用活血补肾壮骨汤,比较两组病例在术后骨折愈合时间,术后3个月骨痂生长评分,术后4、8、12周两组血清中碱性磷酸酶(ALP)及骨钙素(BGP)量的差别等。结果观察组术后骨折愈合时间显著短于对照组(P<0.05),观察组术后3个月骨痂评分显著高于对照组(P<0.05),观察组术后8、12周ALP及BGP明显高于对照组(P<0.05),而术后4周两组间ALP及BGP差异无统计学意义(P>0.05)。结论锁定钢板植骨内固定配合中药活血补肾壮骨汤治疗胫骨中下段骨折不愈合,能促进骨痂生成,提高骨折愈合率。
Objective To explore the efficacy of locking plate internal fixation and bone grafting combined with Chinese medicine treatment on middle and lower tibia fracture nonunion. Methods 86 patients of middle and lower tibia fracture nonunion were divided into two groups(43 per group),all of the patients were chosen from Luoyang Orthopedic Hospital of Henan province.All of the patients were treated with locking plate and autogenous iliac vertebra internal fixation.Meanwhile,cases were combined with Huoxue Bushen Zhuanggu decoction treament.Thereafter,fracture healing time,the score of bone growth after 3 months compared between cases and controls,as well as the differences of serum ALP and BGP levels were observed between the two groups at 4 weeks,8 weeks and 12 weeks after operation. Results Fracture healing time of cases group were significantly shorter than controls(P<0.05);in addiction,the score of bone growth at 3 months after the operation of cases were much higher than controls(P<0.05).Serum ALP and BGP levels of cases at 8 weeks and 12 weeks after operation were significantly higher than controls;however,there were no differences between groups of serum ALP and BGP levels at 4 weeks after operation(P>0.05). Conclusion Locking plate internal fixation and bone grafting combined with Chinese medicine treatment on middle and lower tibia fracture nonunion can promote callus formation and improve fracture healing rate.
