脊髓室管膜瘤和星形细胞瘤的MRI影像诊断 被引量:3

摘要 目的探讨脊髓室管膜瘤和星形细胞瘤在MRI上的影像特征,提高其诊断的准确率。方法回顾性分析经手术病理证实的15例脊髓室管膜瘤和10例星形细胞瘤的临床表现和MRI特征。结果两者均以T_1WI等、低信号,T_2WI等、高信号为主。15例脊髓室管膜瘤呈中心性生长,2例可见肿瘤内出血,4例肿瘤内部见囊变,9例肿瘤实质头端或尾端脊髓反应性囊变,4例肿瘤可见"帽征"。10例星形细胞瘤病例中,6例呈偏心性生长,仅1例可见肿瘤内出血,3例肿瘤内部见囊变,4例肿瘤实质头端或尾端脊髓反应性囊变,未见明显"帽征"。MRI增强扫描室管膜瘤组12例肿瘤边界清楚,5例肿瘤明显均匀强化,6例可见斑片状或结节状强化。星形细胞瘤组7例病例边界不清,仅2例病灶均匀强化,其余8例均表现为斑片状、点状或环状强化。结论脊髓室管膜瘤和星形细胞瘤的MRI表现具有一定的特征性,MRI平扫和增强扫描对两者的的诊断和鉴别诊断具有重要价值。 Objective By evaluating the magnetic resonance imaging(MRI)features of spinal cord ependymomas and astrocytomas to promote the accuracy in diagnosis.Methods The clinical and MRI features of15ependymomas cases and10astrocytomas cases,confirmed by operation and pathology were retrospectively reviewed.Results Most of tumors were iso-or hypo-intensity on T1WI and iso-or hyper-intensity on T2WI.All of15ependymomas located centrically in the spinal cord.Intratumoral hemorrhage and cystic degeneration were found in2and4cases retrospectively.Cranial and/or caudal reactive cysts was found in9cases,and the“cap sign”was seen in4cases.6of10astrocytomas cases grow eccentrically,with just1case of cystic degeneration,3cases of intratumoral hemorrhage,and4cases of cranial and/or caudal reactive cysts,and no case exhibited“cap sign”in the astrocytomas group.On enhanced images,margins were well-defined in12tumors in ependymomas group,in which5ependymomas with obvious and uniform enhancement,6cases with patchy and nodular enhancement.7of the10astrocytomas cases showed unclear boundary,2astrocytomas had uniform enhancement and the other cases had patchy and nodular enhancement.Conclusions The MRI performance of ependymomas and astrocytomas has certain characteristic,and MRI features are helpful for differential diagnosis between the two intramedullary tumors.
