摘要
目的探讨微创肌间隙入路普通椎弓根螺钉在胸腰椎骨折的临床应用。方法选取2013年4月至2016年5月本院就诊胸腰椎骨折患者202例,随机分为两组。对照组采用传统入路普通椎弓根螺钉手术治疗,观察组采用微创肌间隙入路普通椎弓根螺钉手术治疗。观察并记录患者出血量、手术时间、引流量以及住院时间,观察术前与术后两组患者影像学情况。结果观察组患者出血量、手术时间、引流量以及住院时间明显少于对照组,差异有统计学意义(P<0.05);两组术后伤椎前缘高度较术前均显著升高(P<0.05),两组术后伤椎后凸角和矢状面Cobb角较术前均显著减小(P<0.05),但观察组术后伤椎前缘高度、伤椎后凸角和矢状面与对照组术后之间差异无统计学意义(P>0.05)。结论微创肌间隙入路普通椎弓根螺钉与传统入路普通椎弓根螺钉治疗胸腰椎均具有良好疗效,但微创肌间隙入路手术治疗可缩短手术时间,减少出血量以及术后引流量,患者恢复较快,住院时间较短。
Objective To discuss the clinical application of minimally invasive muscle interstitial approach with common ver原tebral pedical screws to thoracolumbar vertebral fracture.Methods202cases of thoracolumbar vertebral fracture patients admitted and treated in our hospital during April2013and May2016were randomly divided into two groups.Patients in the contrast group were given conventional approach with common vertebral pedical screws operation,while patients in the observation group were giv原en minimally invasive muscle interstitial approach with common vertebral pedical screws operation.Observe and record the amount of bleeding,operation time,volume of drainage as well as the length of hospital stay.Observe the iconography condition of before and after operation in both groups.Results The patients'amount of bleeding,operation time,volume of drainage and the length of hos原pital stay in the observation group were significantly less than the contrast group.The differences were significant(P约0.05).The ante原rior height of the injured vertebrae of after operation was significantly higher than that of before operation in both groups(P约0.05).The kyphotic angle and the sagittal plane Cobb angle of the injured vertebrae in both groups were significantly decreased than those of before operation(P约0.05).However,the comparison of the anterior height of the injured vertebrae,kyphotic angle and the sagittalplane Cobb angle of the injured vertebrae between the observation group and contrast group showed no significant difference(P跃0.05).Conclusion Both the minimally invasive muscle interstitial approach with common vertebral pedical screws and conventional approach with common vertebral pedical screws applying to the thoracolumbar vertebral fracture have good curative effects.But mini原mally invasive muscle interstitial approach with common vertebral pedical screws can shorten the operation time,reduce the amount of bleeding and post-operative volume of drainage,accelerate the recovery time and shorten the length of hospital stay.
