摘要
目的比较传统的凿骨劈冠法与高速涡轮机去冠法拔除下颌近中阻生第三磨牙的临床效果。方法将100例下颌近中阻生第三磨牙患者按照随机数字表法分为对照组和观察组,每组50例。对照组采用传统的凿骨劈冠法,观察组采用高速涡轮机去冠法。观察2组拔牙时间和术后疼痛、术后肿胀、术后张口受限、术后感染及颞下颌关节损伤(翼外肌功能亢进、翼外肌痉挛、咀嚼肌痉挛、可复性盘前移位、不可复性盘前移位)发生率。结果观察组拔牙时间明显短于对照组[(21.32±3.42)min比(28.64±4.29)min,P<0.05]。观察组术后疼痛(中度+重度)、术后肿胀(中度+重度)、术后张口受限、术后感染及翼外肌功能亢进、翼外肌痉挛、咀嚼肌痉挛、可复性盘前移位发生率均明显低于对照组(均P<0.05)。结论高速涡轮机去冠法拔除下颌近中阻生第三磨牙,临床效果优于传统的凿骨劈冠法。
ObjectiveTo compare the clinical efficacies of traditional chisel splitting method and high-speed turbine crown removal for the extraction of mesially impacted mandibular third molars.Methods One hundred patients with mesially impacted mandibular third molars were randomly assigned to receive either traditional chisel splitting method(control group,n=50)or high-speed turbine crown removal(observation group,n=50).Tooth extraction time,postoperative pain,postoperative swelling,postoperative mouth opening restriction,postoperative infection and temporomandibular joint injury(pterygoid hyperfunction,lateral spasticity,masticatory muscle spasm,recoverable disc displacement and non-recoverable disc displacement)were observed in both groups.Results The tooth extraction time in observation group was significantly shorter than that in control group((21.32±3.42)minutes vs(28.64±4.29)minutes,P<0.05).The incidences of postoperative pain(moderate+severe),swelling(moderate+severe),mouth opening restriction,infection,pterygoid muscle hyperactivity,pterygoid muscle spasm,masticatory muscle spasm and reducible anterior disc displacement in observation group were significantly lower than those in control group(P<0.05).Conclusion High-speed turbine crown removal is superior to traditional chisel splitting method for extracting mesially impacted mandibular third molars.
