摘要
目的探讨消化内镜技术在上消化道早癌临床诊断与治疗中的应用价值。方法选取2015年10月至2016年12月我院收治的85例上消化道早癌患者进行临床研究,按诊疗方法不同分为观察组(45例)与对照组(40例)。对照组行传统内镜检查+外科手术切除术治疗,观察组行高清内镜检查+内镜下黏膜切除术(EMR)治疗,比较两组的临床诊治效果。结果观察组经高清内镜检查后,形态影像、胃小凹分型影像及毛细血管影像评分均明显高于常规内镜检查的对照组(P<0.01)。观察组的平均手术时间、住院时间均显著短于对照组(P均<0.01),且观察组治疗费用显著少于对照组(P<0.01)。观察组术后并发症发生率为8.89%,显著低于对照组的30.00%(P<0.05)。结论内镜技术(NBI+EMR)诊治上消化道早癌具有诊断率较高、治疗时间短、并发症少及治疗费用低等优点。
Objective To explore the value of digestive endoscopy in the clinical diagnosis and treatment of early cancer of upper digestive tract.Methods From October2015to December2016,85patients with upper digestive tract cancer who were treated in our hospital were selected for clinical trial.The patients were divided into observation group(n=45)and control group(n=40)according to different diagnosis and treatment strategy.The control group received conventional endoscopy plus surgical resection,and the observation group underwent high-endoscopic and endoscopic mucosal resection(EMR).The clinical diagnosis and treatment results were compared between the two groups.Results After high-definition endoscopic examination,the morphological image,foveoscopic image and capillary angiography were significantly higher in the observation group than those in the conventional endoscopy group(P<0.01).After the clinical treatment,the mean The operation time and hospitalization time were significantly shorter than those in the control group,and the treatment cost in the observation group was significantly less than that in the control group(P<0.01).The incidence of postoperative complications in the observation group(8.89%)was significantly lower than that in the control group(30.00%)(P<0.05).Conclusion Endoscopic technique(NBI+EMR)has the advantages of higher diagnosis rate,shorter treatment time,less complications and lower treatment cost in the diagnosis and treatment of early cancer of upper digestive tract.
