摘要
目的探讨改良根向复位瓣术对种植二期手术患者实施附着龈重建治疗的临床修复效果。方法回顾性分析本院2013年12月~2014年12月收治的13例行种植二期手术和同期附着龈重建治疗患者的临床资料,均利用改良根向复位瓣术进行附着龈重建。结果入组的13例患者一共有15颗种植体接受治疗,术后14 d进行复查并制取印模的时候,经复查发现,15个种植位点中有13个黏膜均呈现愈合良好现象,其余2个黏膜发生炎症。术后1个月复查,在进行牙冠佩戴的时候,正常角化黏膜均良好形成。在有效角化黏膜宽度方面,治疗前,13例患者的平均为有效角化黏膜宽度为(0.19±0.44)mm,治疗后1个月复查,较之治疗前,宽度得到显著的增加,达到(2.61±0.46)mm,差异有统计学意义(P<0.05);治疗后6个月进行复查,较之治疗后1个月,宽度得到显著增加,达到(2.64±0.54)mm,差异有统计学意义(P<0.05),冠修复6个月后进行复诊,可以发现患者均处于良好的口腔卫生状况。患者的种植体均处于良好的稳定状态。重建角化黏膜与烤瓷冠紧密贴合,且未出现明显炎症。经牙周复查,各项指标均处于良好水平,其中改良菌斑指数为(0.71±0.81),出血指数为(0.74±0.63),探诊深度为(1.81±0.35)mm。结论改良根向复位瓣术应用于种植二期手术患者附着龈重建治疗之后可以获得良好的临床修复效果。
Objective To investigate the clinical effect of modified radical reduction flap in the reconstruction of two patients undergoing reconstruction of the attached gingiva.Methods The clinical data of13patients who underwent two stage operation and the same period of reconstruction were analyzed retrospectively.All patients were treated with modified radical reduction flap for reconstruction of the attached gingiva in December2013and December2014.Results13cases were enrolled a total of15implants for treatment,postoperative14d was reviewed and impressions,the review found that15implant sites in13mucosa showed good healing phenomenon,the remaining2mucosal inflammation.1months after the operation,the normal keratinized mucosa was formed when the crowns were worn.The effective width of keratinized mucosa,13cases of patients before treatment,the average for the effective width of keratinized mucosa(0.19±0.44)mm,1months after treatment were compared before treatment,the width was increased to(2.61±0.46)mm,the difference was statistically significant(P<0.05);review6months after treatment,compared with1months after treatment,the width increased significantly,reaching(2.64±0.54)mm,the difference was statistically significant(P<0.05),crown after6months of follow-up,can be found in patients with good oral hygiene status.The implants were in stable condition.Reconstruction of keratinized mucosa and porcelain fused to the crown,and no obvious inflammation.After the periodontal examination,all the indexes were in good level,the improved plaque index was(0.71±0.81),and the bleeding index was(0.74±0.63),and the depth of diagnosis was(1.81±0.35)mm.Conclusion Modified radical reduction flap technique can be used in two stage operation for patients who have undergone reconstruction of gingival reconstruction.Good clinical results can be obtained after reconstruction.
