摘要
目的探讨对分化型甲状腺癌患者采用不同手术方式治疗的临床效果,并分析影响治疗效果的因素。方法随机选择208例患者作为本次研究对象,均为2014年12月~2016年12月期间收治的分化型甲状腺癌患者,依据手术方式将208例患者分为4组,每组各52例,其中A组患者采用甲状腺全切术治疗,B组患者在A组患者甲状腺全切治疗的基础上,增加单双侧颈淋巴结清扫;C组患者采用患侧腺叶切除即峡部切除治疗,D组患者在C组患者患侧腺叶切除及峡部切除治疗的基础上,增加对侧大部切除术治疗;对4组患者的临床疗效进行比较,并通过随访观察影响治疗效果的因素。结果 A组、B组、C组、D组患者治疗有效率分别为96.15%、96.15%、96.15%、94.23%,数据比较差异无统计学意义;A组患者并发症发生率为19.23%,B组患者并发症发生率为28.85%,C组患者并发症发生率为9.62%,D组患者并发症发生率为15.38%;数据比较差异均无统计学意义;不同性别、不同年龄、不同病理类型及不同肿瘤分期比较差异均具有统计学意义(P<0.05)。结论 4种手术方式治疗分化型甲状腺癌患者效果差异无统计学意义,均具有一定的疗效,而影响治疗效果因素主要为肿瘤病理类型及分期,具有重要的研究价值。
Objective To investigate the clinical effect of patients with differentiated thyroid carcinoma treated by different operative ways,andanalyzes factors influencing the therapeutic effect.Methods208cases of randomly selected patients as the research object,all patients with differentiatedthyroid cancer treated during December2014to December2016,according to the operation procedure,208patients were divided into the4group,52cases in each group,the A group were treated with total thyroidectomy for the treatment group,B patients with total resection in the treatmentof the patients in the A group on the basis of the thyroid,increasing bilateral cervical lymph node dissection;C patients with ipsilateral lobectomyor isthmus resection,group D Patients in group C patients with ipsilateral lobectomy or isthmus resection treatment,increased contralateral subtotalresection in the treatment of;to compare the clinical efficacy of the four groups,and the factors affect the treatment effect observed by follow up.Results In A group,B group,C group,D group,the effective rate of treatment were96.15%,96.15%,96.15%,94.23%,the difference between datawere not statistically significant;A group of patients the incidence of complications was19.23%,patients in the B group the incidence of complicationswas28.85%,patients in the C group the complication rate was9.62%of patients in the D group,the complication rate was15.38%;the datashowed no significant difference;different gender,different age,different types and different cases of tumor staging were statistically significant(P<0.05).Conclusion The effect of different treatment of differentiated thyroid carcinoma in four surgical patients obviously,have certain curative effect,the treatment effect is a major factor in tumor pathological type and stage,it has important research value.
