动态动脉硬化指数、高同型半胱氨酸血症与高血压患者早期肾功能损害的关系 被引量:12

The correlation between the ambulatory arterial stiffness index and high homocysteine with early renal dysfunction in essential hypertension patients

摘要 目的探讨原发性高血压(EH)患者动态动脉硬化指数(AASI)、高同型半胱氨酸血症(Hhcy)与早期肾功能损害的关系。方法入选2010年1月~2015年9月在中国民航总医院心内科就诊的原发性高血压患者319例。依据动态动脉硬化指数正常值≤0.55和同型半胱氨酸正常值≤15μmol/L,将入选患者分为:A组为Hhcy组(n=83),其中男性48例;B组为异常AASI组(n=84),男性54例;C组为Hhcy组+异常AASI组(n=80),其中男性56例及D组单纯高血压组(n=72),男性44例。所有入选患者均行24h动态血压监测、血清同型半胱氨酸、肾功能、血糖、血脂和尿常规的检测。结果①估计的肾小球滤过率(eGFR)水平比较:与D组相比,A组、B组、C组eGFR明显下降差异具有统计学意义(P<0.05)。与C组相比,A组、B组eGFR明显下降,差异有统计学意义(P<0.05)。②尿蛋白阳性率比较:与D组比较,C组、A组、B组尿蛋白阳性率明显增加,差异有统计学意义(P均<0.001)。③eGFR有关因素分析:相关分析显示,年龄、病程、AASI、Hhcy与eGFR呈负相关。多元线性回归分析发现,年龄、Hhcy及AASI仍与eGFR独立相关。结论AASI异常、Hhcy分别与高血压患者早期肾功能损害独立相关,且异常AASI合并Hhcy对高血压患者早期肾功能损害更显著。 Objective To investigate the relationship between ambulatory arterial stiffness index (AASI) and High homocysteine (Hhcy) with early renal function injure in essential hypertension (EH) patients.Methods A total of 319 EH patients of Aviation General Hospital from Jan. 2010 to Sep. 2015 were enrolled. According to AASI normal value (≤0.55) and Hhcy normal value (≤15μmol/L), patients were divided into four groups: A group, also called Hhcy group, has 83 patients including 48 males; B group, also called AASI group, has 84 patients including 54 males;C group, also called Hhcy + abnormal AASI group, has 80 patients including 56 males; D group, also called EH group,has 72 patients including 44 males. All groups were received 24 h ambulatory blood pressure monitoring, examination of serum homocysteine, renal function, blood glucose, blood fat, and urine routine test. Results ①Estimated glomerular filtration rates (eGFR) in A group, B group and C group were significantly lower than D group (P<0.05), while eGFR were significantly lower in C group than A group and B group (P<0.05). ②Levels urinary protein were significantly higher in A group, B group and C group than D group (P<0.001). ③Result of correlation analysis showed a negative correlation between eGFR with age, process, AASI and Hhcy. Result of multivariate linear regression analysis showed eGFR was independently related with age, Hhcy or AASI. Conclusion Abnormal AASI and Hhcy are associated with early renal dysfunction in patients with EH respectively, and abnormal AASI combined with Hhcy is more significantly associated with early renal dysfunction in patients with EH.
