摘要
目的探讨分析^(18)F-FDG PET/CT在结肠癌N分期中的诊断价值。方法选取本院收治的62例结肠癌患者作为研究对象,对患者术前^(18)F-FDG PET/CT检查结果与术后病理检查结果实施回顾性分析,并且按照解剖学定位对结肠癌区域淋巴结转移情况进行统计。结果同机64层CT以5、8、10 mm作为参考界值,诊断的灵敏度为85.2%、39.7%、52.2%,特异度为62.7%、88.1%、81.0%,准确性为33.6%、94.2%、75.1%;以SUVmax 1.0、2.0、3.0为参考界值,诊断的灵敏度为100%、37.8%、55.3%,特异度为90.3%、83.4%、85.2%,准确性为78.2%、95.6%、90.7%。两者结合对直肠癌的N分期的诊断准确性能够达到90%。结论利用^(18)F-FDG PET/CT对结肠癌患者的N分期进行诊断,具有较高的确诊率,能够清晰显示区域淋巴结情况。结肠癌N分期的评判标准为淋巴结短径5~8 mm、SUVmax取值在2.0~3.0,能够显著加强PET/CT在结肠癌术前分期的准确性。
Objective To evaluate the diagnostic value of 18F-FDG PET/CT in N staging of colon cancer.Methods A retrospective analysis of the preoperative 18F-FDG PET/CT results and postoperative pathological results of the 62 colon cancer patients admitted was conducted,and the regional lymph node metastasis of colon cancer was statistically analyzed according to the anatomical location.Results The sensitivity of the 64-slice CT using5,8 and 10 mm as the reference value were 85.2%,39.7%and 52.2%,the specificity were 62.7%,88.1%,81.0%,and the accuracy were 33.6%,94.2%and 75.1%.The sensitivity,of the diagnosis were 100%,37.8%and 55.3%,the specificity were 90.3%,83.4%,85.2%,and the accuracy were 78.2%,95.6%and 90.7%.The combined diagnosis accuracy of N staging of rectal cancer can reach 90%.Conclusion The diagnosis method of 18F-FDG PET/CT in patients with N staging of colon cancer has a high rate of disease diagnosis and can clearly show regional lymph nodes.The evaluation criteria of N staging of colon cancer are short diameter of 58 mm,and the value of SUVmax betwee n2.03.0,which can significantly enhance the accuracy of PET/CT in preoperative staging of colon cancer.
