摘要
目的探讨血液透析患者心血管事件与瓣膜钙化和血压变异性之间的关系，分析其影响因素。方法以2015年至2017年在我院透析中心82例接受规律血液透析的患者为研究对象，心血管事件组27例，无心血管事件组55例。检查心脏彩超、腹主动脉钙化积分、血压变异性、生化指标，应用Logistic回归分析探讨以上参数的相关性。结果瓣膜钙化、腹主动脉钙化积分、糖尿病、平均实际血压变异（ARV）是心血管事件的高危因素（P〈0．05）。年龄、ARV是瓣膜钙化的高危因素（P〈0．05）。ARV较收缩压标准差能更好地反映心血管事件的风险。结论心脏瓣膜、血管钙化，ARV、糖尿病是透析患者心血管事件高危因素。ARV是瓣膜钙化高危险因素。
Objective To investigate the relationship between cardiovascular events and valve calcification and blood pressure variability in hemodialysis patients. Methods From 2015 to 2017, there were 82 regular hemodialysis patients treated in the Dialysis Center of our hospital, 27 cases of cardiovascular events and 55 cases of no cardiovascular events. Cardiac color ultrasound, abdominal aortic calcification score, blood pressure variabihty, biochemical indicators and other monitoring were conducted to analyze the correlation. Logistic regression analysis of cardiovascular events and valve calcification was performed. Results Valve calcification, abdominal aortic calcification score, diabetes, average real blood pressure variation （ARV） were high risk factors for cardiovascular events. Age and ARV are high risk factors for valve calcification. ARV better reflects the risk of cardiovascular events than standard deviation of systolic blood pressure. Conclusions Cardiac valve, vascular calcification, ARV and diabetes are high risk factors of cardiovascular events in dialysis patients. ARV is a high risk factor for valve calcification.
