摘要
目的探讨腮腺上皮源性肿瘤超声声像图特点,并结合其病理组织学特点,为超声定性诊断及鉴别肿瘤的良恶性提供依据。方法回顾分析我院2013年1月～2017年6月经病理证实的102例腮腺上皮源性肿瘤,分析其超声声像图特征,比较不同良恶性肿瘤及不同病理特点肿瘤的声像图差异。结果 102例病例中,肿块的超声检出率为100%,良性88例,恶性14例。其中多形性腺瘤38例,腺淋巴瘤30例,基底细胞腺瘤9例,淋巴上皮囊肿6例,肌上皮瘤3例,良性淋巴上皮病变2例,鳞癌5例,基底细胞腺癌1例,肌上皮癌1例,腺样囊性癌3例,腺泡细胞癌3例,腮腺淋巴上皮样癌1例。超声显示不同病理特点的肿瘤内部回声存在差异。肿瘤的形态和边界、伴钙化、同侧颈部淋巴结转移在良恶性组之间比较,差异有统计学意义(P <0. 05)。结论高频超声可以有效的发现腮腺肿瘤性病变,对于肿瘤的诊断及鉴别诊断具有重要意义。
Objective To investigate the ultrasonographic features of parotid epithelial tumors and to provide basis for the qualitative diagnosis and differential diagnosis of benign and malignant tumors by combining the histopathological features of parotid gland. Methods We retrospectivlly analysed 102 cases of parotid epithelial tumors confirmed by pathology in our hospital from January 2013 to June 2017, analyzed the sonographic features, compared the difference of ultrasonographic features of benign and malignant tumors and different pathological characteristics of tumors. Results In 102 cases, the ultrasonic detection rate was 100%, （benign in 88 cases, malignant in 14 cases）. There were 38 cases of pleomorphic adenoma, 30 cases of adenolymphoma, 9 cases of basal cell adenoma, 6 cases of lymphoepithelial cyst, 3 cases of myoepithelioma, 2 cases of benign lymphoepithelial lesion, 5 cases of squamous cell carcinoma, 1 case of basal cell adenocarcinoma, 1 case of myoepithelial carcinoma, 3 cases of adenoid cystic carcinoma, 3 cases of acinic cell cancer, and 1 case of branchial gland lymphoepithelial carcinoma. Ultrasound showed that there were differences in internal echo of tumor with different pathological characteristics. There were significant differences between the benign and malignant group in the shape and border of the tumor, calcification, and lymph node metastasis of the ipsilateral neck （ P 〈0.05）. Conclusion High frequency ultrasound can effectively detect parotid tumor lesions, which is of great significance for the diagnosis and differential diagnosis of tumors.
