东莞市石碣镇小学生牙窝沟封闭项目的封闭率调查及封闭剂保留的影响因素分析 被引量:6

摘要 目的调查东莞市石碣镇实施小学生牙窝沟封闭项目的封闭率,并探讨封闭剂脱落的影响因素。方法采用随机抽样方法选择2017年东莞市石碣镇参加免费窝沟封闭防龋项目的 14所小学二年级学生2195例为研究对象。对2017年东莞市石碣镇小学生牙窝沟封闭项目情况进行调查分析,在封闭完成后3、6、12个月进行复查窝沟封闭剂的保留情况,并对可能影响封闭剂保留的相关因素进行分析。结果本组研究完成窝沟封闭人数2195例(窝沟封闭牙齿数6347颗),封闭率为55.14%。本组窝沟封闭术后12个月复查发现封闭剂脱落的牙数699颗,脱落率为11.01%。单因素分析,年龄、刷牙次数、封闭人员职称、饮用碳酸饮料频率、乳牙龋补面数、牙位与封闭剂脱落率密切相关(P <0.05),而患儿其他临床指标与其封闭剂脱落率无明显相关性(P> 0.05)。多因素逐步Logistic回归结果发现年龄(OR=0.817)、刷牙次数(OR=0.307)、乳牙龋补面数(OR=1.722)是影响封闭剂脱落率的独立危险因素,其中年龄和刷牙次数是保护因素。结论临床应重视乳牙萌出,减少乳牙的患龋率,关注年龄小儿童的随访和复查,加强健康教育,引导学龄儿童建立良好的刷牙习惯,降低窝沟封闭剂的脱落率,进而提高窝沟封闭防龋效果。 Objective To investigate the closure rate of pit and fissure sealing program for primary school students in Shijie town of Dongguan and to explore influencing factors for sealant dropping. Methods 2195 second-year students from 14 primary schools who took part in the free pit and fissure sealing in prevention of caries program were selected as research objects by the random sampling method.Pit and fissure sealing program for primary school students in Shijie town of Dongguan in 2017 was investigated and analyzed.In 3,6,12 months after sealing,the retention of pit and fissure of sealant was rechecked and related factors were analyzed. Results A total of 2195 patients (6,347 teeth) who were given pit and fissurre sealing and the closure rate was 55.14%.In reexamination of 12 months after pit and fissure sealing,699 teeth were with sealant dropping and the drop rate was 11.01%.The univariate analysis showed that age,times for brushing teeth,sealing staff title,frequency of drinking carbonated drinks,deciduous teeth decay missing filling surface and tooth position were significantly related to drop rate of sealant (P 〈 0.05),while other clinical indicators and drop rate of sealant had no obvious correlation (P 〉 0.05).Muhivariate Logistic regression analysis showed that age (OR=0.817),times for brushing teeth (OR=0.307) and deciduous teeth decay missing filling surface (OR=1.722) were independent risk factors affecting drop rate of sealant.Age and times for brushing teeth were protective factors. Conclusion The clinic should pay attention to eruption of deciduous teeth and reduce the caries rate of deciduous teeth.They need focus on follow-up and review of young children,strengthen health education,guide the school-age children to establish good habits of brushing teeth and reduce drop rate of pit and fissure sealant to improve the effect of pit and fissure sealant on preventing caries.
