摘要
目的应用脑象图技术(EEQG)探讨与抑郁症诊断相关的脑象图指标及其大脑区域,为抑郁症临床诊断提供客观依据;评价脑象图技术在抑郁症诊断中的价值。方法选取抑郁症患者(抑郁症组) 30例和正常对照(对照组) 31例,应用脑象图技术筛查,进行对照分析与诊断评价。结果 16导联频率中抑郁症组双侧额极区、双侧额区频率均低于对照组(P <0. 01),16导联功率中抑郁症组双侧额极区、双侧额区、双侧前颞功率均高于对照组(P <0. 01)。抑郁症组右额区耗能值高于对照组(P <0. 05),右额区、左中央区、右前颞区过稳态值均高于对照组(P <0. 05),右额区、左前颞区、左中颞区抑郁值均高于对照组(P <0. 05),右前颞区逃逸值、右前颞区焦虑值高于对照组(P <0. 05,P <0. 01)。脑象图临床检查结果的符合率为86. 89%,Kappa值为0. 74。灵敏度为86. 67%,特异度为87. 10%,阳性似然比为6. 72,阴性似然比为0. 15,Youden指数为0. 74,AUC为0. 910。结论脑象图作为抑郁症临床辅助诊断的脑生理学指标,具有一定的临床指导意义。
Objective To provide evidence for clinical diagnosis of depression by investigating the index of electroencephaloquadrantogram （EEQG） and brain regions related to the diagnosis of depression. To evaluate the value of EEQG in the diagnosis of depression. Methods A total of 30 major depressive disorder （MDD） patients （MMD group） and 31 healthy controls （control group） were selected and screened by EEQG. The data between two groups were compared. Results The frequencies of bilateral frontal pole and bilateral frontal area in MDD group were significantly lower than those in control group（ P 〈 0.01 ）. The powers of bilateral frontal pole, bilateral frontal area and bilateral anterior temporal lobe in MDD group were significantly higher than those in control group（P 〈 0.01 ）. Compared with those in control group, the energy consumption of F4, over-steady value of F4, F8 ,C3, depression value of F4, FT, T3, escape value of F8 and anxiety value of F8 were higher in MDD group（P 〈 0.05 ）. There were no significant differences of other values between two groups. The coincidence rate of EEQG was 86.89% , with the Kappa value 0.74. The sensitivity and specificity were 86.67% and 87.10% , respectively. The positive and negative likelihood ratio were 6.72 and 0.15, respectively. The Youden index was 0.74 and AUC was 0. 910. Conclusion EEQG has guiding significance for the diagnosis of depression as an ancillary tool.
