肺结核病患者外周血单个核细胞中环状RNA circRNF10的表达及意义 被引量:10

Expression and clinical significance of circular RNA circRNF10 in peripheral blood mononuclear cells from patients with tuberculosis

摘要 目的检测肺结核病患者外周血单个核细胞(PBMC)中环状RNA circRNF10的表达水平,探讨其在肺结核病诊断中的应用价值。方法采用实时荧光定量PCR法对2016年3月至2017年3月江西省胸科医院收治的90例未经治疗的活动性肺结核患者、40例肺炎患者、40例肺癌患者及70例健康体检者PBMC中circRNF10的表达水平进行检测。应用受试者工作特征曲线(ROC)曲线分析circRNF10诊断肺结核的敏感度和特异度。结果肺结核患者PBMC中circRNF10表达水平(2.11±1.13)显著高于健康对照组(1.00±0.38)、肺炎组(1.14±0.57)和肺癌组(0.93±0.31),差异有统计学意义(F=38.15,P<0.01)。肺结核患中肺部空洞组circRNF10水平显著高于无空洞组(t=7.02,P<0.01);肺部病灶范围≥2个肺野组circRNF10水平显著高于<2个肺野组(t=4.68,P<0.01)。与抗结核治疗前(2.31±0.72)相比,肺结核患者circRNF10表达水平在抗结核治疗后3月(1.27±0.41)(t=5.62,P<0.01)、6月(0.94±0.38)(t=7.54,P<0.01)均有显著降低。ROC曲线分析显示,circRNF10在鉴别肺结核与健康对照、肺炎患者及肺癌患者的曲线下面积(AUC)分别为0.837、0.782、0.861。结论 PBMC中circRNF10有望作为肺结核诊断和疗效监测的潜在生物标志物。 Objective To detect circular RNA(circRNA) circRNF10 level of peripheral blood mononuclear cells(PBMC) in pulmonary tuberculosis(TB) patients,and evaluate its diagnostic value for TB. Methods From March 2016 to March 2017,circRNF10 level of PBMC were detected by real-time quantitative polymerase chain reaction(qRT-PCR) in 90 patients with TB who were hospitalized in Jiangxi Chest Hospital,40 patients with pneumonia,40 patients with lung cancer and 70 control subjects. The sensitivity and specificity of circRNF10 were analyzed by the receiver operating characteristic(ROC) curve. Results The circRNF10 level of PBMC in TB patients(2.11±1.13) were significantly higher than those in patients with pneumonia(1.14±0.57),lung cancer(0.93±0.31) and normal controls(1.00±0.38)(F=38.15,P0.01). The circRNF10 level of PBMC in cavity pulmonary TB patients were higher than that in pulmonary TB patients without cavity(t=7.02,P0.01). The circRNF10 level of PBMC in TB patients with involvement of ≥2 lung fields were higher than 2 lung fields(t=4.68,P0.01). As compared to their pre-treatment(2.31±0.72),the circRNF10 level decreased significantly in 3 months(1.27±0.41)(t=5.62,P0.01) and 6 months(0.94±0.38)(t=7.54,P0.01) after anti-TB therapy. The area under the ROC curve(AUC) of circRNF10 in the discriminated the patients with TB from normal controls,pneumonia patients and lung cancer patients were 0.837,0.782 and 0.861,respectively. Conclusion The circRNF10 lever of PBMC could be used as a potential biomarker in TB diagnosis and monitoring.
