摘要
目的分析北京市朝阳区危重新生儿救治体系运行10年的成效,为进一步完善区域性危重新生儿转运救治提供参考。方法对朝阳区2007-2016年26家助产机构新生儿救治状况进行回顾性分析。结果朝阳区10年来户籍活产数175 713例,早期新生儿死亡209例,占5岁以下儿童死亡的36.35%。2007年救治体系建立初期,早期新生儿、新生儿、婴儿以及5岁以下儿童的死亡率分别是1.39‰、2.09‰、2.71‰、3.49‰,2016年分别为1.13‰、1.41‰、2.13‰、2.54‰,5岁以下儿童各年龄别死亡率均呈逐年下降趋势,差异有统计学意义(P<0.05)。10年共转运危重新生儿5 508例,转运数量前3位是早产儿、低血糖、新生儿窒息,其中早产儿转运最多占52.68%。结论降低早期新生儿死亡率是降低婴儿死亡率的关键,建立和完善危重新生儿救治体系是降低新生儿死亡率的保障。建立健全覆盖区域性的危重新生儿救治体系,对提高整个地区新生儿存活率、降低新生儿死亡率具有重要意义。
Objective To analyze the achievement of regional critical newborn treatment system in Chaoyang District of Beijing from2007 to 2016,provide reference for further improvement of regional critical newborn treatment and transport. Methods A retrospective analysis was conducted to analyze neonatal treatment in 26 midwifery institutions in Chaoyang District from 2007 to 2016. Results From2007 to 2016,there were 175 713 cases of live births in Chaoyang District,209 cases of early neonatal death were found,accounting for36. 35% of the children death under 5 years old. In the early stage of establishment of regional critical newborn treatment system in 2007,the mortality rates of early neonates,neonates,infants,and children under five years old were 1. 39‰,2. 09‰,2. 71‰,and 3. 49‰,respectively,which were 1. 13‰,1. 41‰,2. 13‰,and 2. 54‰ in 2016,respectively. Age-specific mortality rates in children under five years old showed decreasing trends year by year（ P〈0. 05）. From 2007 to 2016,5 508 critical neonates were transported totally,the top three causes of neonatal transport were premature birth,hypoglycemia,and neonatal asphyxia,the proportion of transport of premature infants accounted for 52. 68%. Conclusion The key point to reduce infantile mortality rate is reducing mortality rate of early neonates,establishing and perfecting regional critical newborn treatment system can reduce mortality rate of neonates,which is of important significance in increasing neonatal survival rate in the region and reduce the mortality rate of neonates.
