DWI显示正常骶髂关节的最佳b值的研究 被引量:2

摘要 目的:通过比较正常骶髂关节在磁共振不同b值的弥散加权成像(DWI)表现,分析其ADC值的差异,探寻1.5 T磁共振DWI显示骶髂关节的最佳b值,为后期研究强直性脊柱炎(AS)骶髂关节炎奠定基础。方法:随机选择20例(男14例,女6例)健康志愿者,年龄15~35岁,对其行骶髂关节的常规MRI扫描及不同b值(0、400、600、800 s/mm^2) DWI轴位扫描,观察3种b值时骶髂关节的显示情况,分别测量3种b值时两侧骶髂关节面下骨髓区(骶骨侧、髂骨侧) ADC值,比较不同b值时两侧骶髂关节面下骨髓区ADC值的差异。结果:对比分析后发现,b值为400 s/mm2及600 s/mm^2时,图像较清晰,骶髂关节与周围组织对比良好; b值为800 s/mm^2时,骶髂关节显示欠清晰,与周围组织对比度下降。两侧骶髂关节面下骨髓区的ADC值随着b值的增高而逐渐减小。3种b值下右髂侧、右骶侧、左骶侧以及左髂侧关节面下骨髓区ADC值的差异均有统计学意义。3种b值下两侧髂骨与骶骨关节面下骨髓区ADC值间差异无统计学意义。b值为400 s/mm^2时,两侧骶髂关节面下骨髓区ADC值的离散度较大; b值为600、800 s/mm^2时,ADC值离散度相对较小。结论:b值为600 s/mm^2时,DWI图像较清晰,骶髂关节面下骨髓区ADC值离散度小、精准可靠,是1.5 T磁共振DWI显示骶髂关节的适宜b值。 Objective :To determine the optimal h-value of 1.5 T magnetic resonance(MR) diffusion weighted imaging(DWI) and analyze the difference of apparent diffusion coefficient(ADC) in showing the normal sacroiliac joint for evidence in following research of ankylosing spondylitis and sacroiliac conditions. Methods :Twenty healthy volunteers aged 15 to 35 years( 14 males and 6 females) were randomly recruited, and underweat conventional MRI scanning of the sacroiliac joint in DWI using different h-value( b-0,400,600,800 s/mm2, respectively)to observe the sacroiliac joint presentation in the different thine h-value. The ADC value of the hone marrow area(sacral side and ilium side ) of the sacroiliac joint was nleasured under the thine h-values, and the difference was compared. Results : b-value at 400 s/mm2 and 600 s/mm2 revealed clear image and better contrast of the sacroiliac joint against the surrounding tissue, However,the sacroiliac join was poorly presented and the contrast against peripheral tissue became weaker under b-value at 800 s/ mm2. The ADC value was gradually decreased with increase of b-value at the bilateral sacroiliac joint surface of bone marrow area. The dift)rence was sig- nificant in ADC value under the three b-value among the right iliac side, the right sacral side, the left sacral side and the left iliac lateral join,yet the difference was insignificant in the ADC value between the two sacral and iliac joints. The dispersion of the ADC value was enlarged at b-value d 400 S/mm2 for bilateral sides of the sacroiliac joint surface of bone marrow area, and became smaller at b-value of 600 s/mm2 and 800 s/mm2. Conclusion : Clearer DWl can be obtained at b-value of 600 s/mm2 ,and dispersion of ADC value is smaller. The findings indicate that 600 s/mm2 should be the optimal b-value in 1.5 T magnetic resonance DWI for sacroiliac joint.
