摘要
目的探讨联合计算机辅助手术(CAS)系统治疗晚期肾癌并发下腔静脉瘤栓的临床效果。方法2017年于我院拟接受手术治疗的晚期肾癌并下腔静脉瘤栓病人1例,利用CAS系统进行术前规划和术中导航,观察CAS系统辅助下治疗晚期肾癌并下腔静脉瘤栓的临床安全性和有效性。结果成功施行肾癌根治术、下腔静脉瘤栓切除术、肾静脉瘤栓切除术、心房切开瘤栓切除术,取出一长约25cm、直径约3cm的癌栓。手术用时8h,术中出血约3 000mL,术中回收式自体输血1 200mL,另输注红细胞13U,输注血浆1 550mL。术后随访12个月,病人恢复良好。结论对于晚期肾癌并下腔静脉瘤栓病人,联合应用CAS系统实施个体化、精准化手术治疗,可获得良好临床效果。
Objective To investigate the clinical effect of computer-assisted surgery（CAS）system in the treatment of advanced kidney cancer complicated by inferior vena cava tumor thrombus. Methods One patient with advanced kidney cancer complicated by inferior vena cava tumor thrombus who planned to undergo surgical treatment in our hospital in 2017 was enrolled.The CAS system was used for preoperative planning and intraoperative navigation,and the clinical effect and safety of CAS in the treatment of advanced kidney cancer complicated by inferior vena cava tumor thrombus were observed. Results Radical nephrectomy,inferior vena cava thrombectomy,renal venous thrombectomy,and atrial thrombectomy were performed successfully,and a tumor thrombus 25 cm in length and 3 cm in diameter was removed.The surgery lasted for 8 h,with an intraoperative blood loss of 3 000 mL;transfusion of intraoperative autologous blood（1 200 mL）,red blood cells（13 U）,and plasma（1 550 mL）were performed.The patient was followed up for 12 months and achieved good recovery. Conclusion Individualized and precise surgery with the help of CAS system has a good clinical effect in the treatment of patients with advanced kidney cancer complicated by inferior vena cava tumor thrombus.
