血清和肽素及降钙素原对骨折术后患者合并肺部感染早期诊断及预后判断的价值 被引量:2

Detection value and prognosis evaluation of copeptin and procalcitonin in early diagnosis of fracture-associated pulmonary infection after surgery

摘要 目的探讨血清和肽素及降钙素原在骨折术后合并肺部感染患者早期诊断及预后判断的价值,为临床诊疗提供参考依据。方法选取2015年8月-2017年8月入院并经手术治疗的骨折患者120例,分为骨折术后组(67例)、肺部感染组(53例),并将同年进行健康体检者作为对照组(60例);分析3组受试者各实验室指标。结果与对照组相比,肺部感染组血清和肽素及降钙素原水平升高,差异均有统计学意义(P<0.05);与对照组相比,骨折术后组血清和肽素及降钙素原水平差异均无统计学意义(P>0.05);与骨折术后组相比,肺部感染组血清和肽素及降钙素原水平均升高,差异有统计学意义(P<0.05)。在肺部感染组中,死亡组血清和肽素、降钙素原水平明显高于生存组,差异有统计学意义(P<0.05)。结论血清和肽素、降钙素原对骨折术后患者合并肺部感染早期诊断及其预后的判断具有重要价值。 Objective To discuss the application value of detection of copeptin and procalcitonin in early diagnosis and prognosis of fracture-associated pulmonary infections after surgery,so as to provide references for clinical treatment. Methods From Aug. 2015 to Aug. 2017,120 fracture patients with surgery were selected in our hospital. The patients were divided into fracture patients with surgery group( n = 67) and pulmonary infection group( n = 53),and 60 controls for healthy examination in our hospital during the same period were selected as control group. The clinical indexes in the three groups were compared.Results Compared with control group,the levels of copeptin and procalcitonin in pulmonary infection group increased significantly,with the differences statistically significant( P〉0. 05). Compared with the control group,there was no statistical significance on the difference in serum,copeptin and procalcitonin in the fracture group( P〈0. 05). Compared with the postoperative group,serum,copeptin and procalcitonin decreased in the lung infection group,with the differences statistically significant( P〈0. 05). In the lung infection group,the serum and propeptide and procalcitonin in the death group were significantly higher than those in the survival group,with the differences statistically significant( P〈0. 05). Conclusion Serum,copeptin and procalcitonin have important value in the early diagnosis and prognosis of patients with postoperative fracture complicated with pulmonary infection.
