摘要
近年来,脑淀粉样血管病的诊断率逐年增加,并已成为老年人原发性脑出血的第二大病因和痴呆的临床表现之一。目前中医对于该病的叙述甚少,对应脑淀粉样血管病的临床表现,该病可分属于中医中风、痴呆等范畴。张大宁教授根据"心肾相交"理论结合心肾的生理病理,首次提出"心-肾轴心系统"学说,融汇理解心肾相交与神经系统和内分泌系统的反馈机制,指出心肾在人体阴阳平调当中起关键的轴心作用。对于脑淀粉样血管病患者的脑出血和认知功能减退可使用补肾活血法治疗。基于"心-肾轴心系统"学说认识脑淀粉样血管病的病因病机,可以为深入了解该病,更好地进行中医治疗提供思路。
In recent years, the diagnostic rate of cerebral amyloid angiopathy has increased year by year, and has become the second major cause of senile primary cerebral hemorrhage and an important sign of dementia. At present, there is little description of the disease in Chinese medicine. Corresponding to the clinical manifestations like cerebral hemorrhage and cognitive dysfunction,the cerebral amyloid angiopathy can be classified into Chinese medicine like apoplexy, dementia and so on. Professor ZHANG Daning first put forward the theory-"heart kidney axis system", which combines the TCM theory and modern medical knowledge. The theory integrates the understanding between heart and kidney and nervous system and the endocrine system feedback mechanism. In addition,the theory points out that the heart and kidney play a key role in the balance of yin and yang. In patients with cerebral amyloidosis, cerebral hemorrhage and cognitive impairment can be treated with tonifying kidney and activating blood. Based on the "heart kidney axis system" theory, we can understand the etiology and pathogenesis of cerebral amyloid angiopathy, so as to provide insights for better understanding of the disease and thus better treatment of Chinese medicine.
