摘要
目的分析昏迷患者的院前急救治疗措施,为提高昏迷患者的院前急救水平提供参考。方法方便选取2017年1—12月该急救中心院前急救的昏迷患者602例进行分析。所有患者的格拉斯哥昏迷量表评分均在3~8分之间,临床诊断为昏迷。对602例昏迷患者的院前急救治疗情况进行回顾性分析,总结昏迷患者的院前急救治疗措施。结果602例昏迷患者经临床诊断,其中262例为急性脑血管疾病,占比43.52%;184例为外伤与中毒患者,占比30.56%;101例为代谢性疾病,占比16.78%;55例为其他疾病,占比9.14%。602例昏迷患者经院前急救治疗后,193例患者好转,325例患者稳定,43例患者病情恶化,25例患者死亡,救治总有效率为86.05%(518/602)。其中死亡率最高的为急性脑血管疾病引起的昏迷。结论昏迷患者往往病情较重,病情进展速度较快,死亡率较高,这就要求院前急救人员要在较短的时间内快速地对患者的病情做出判断,并给予有效的初步急救治疗,才能够确保昏迷患者获得良好的急救治疗效果,有效改善昏迷患者的预后。
Objective To analyze the prehospital emergency treatment measures for coma patients and to provide reference for improving the prehospital emergency level for coma patients. Methods A total of 602 coma patients from the hospital in January to December 2017 were convenient selected for analysis. All patients had a glasgow coma scale score of between 3 and 8 points, with a clinical diagnosis of coma. A retrospective analysis of pre-hospital emergency treatment of 602 coma patients was conducted to summarize prehospital emergency treatment measures for coma patients. Results 602 cases of coma patients were diagnosed clinically. 262 cases were acute cerebrovascular diseases, accounting for 43.52%; 184 cases were traumatic and poisoned patients, accounting for 30.56%; 101 cases were metabolic diseases, accounting for 16.78%; 55 cases for other diseases, accounting for 9.14%. After 602 unconscious patients were treated with pre-hospital care, 193 patients improved, 325 patients were stable, 43 patients deteriorated, and 25 patients died. The total effective rate was 86.05%（518/602）. The highest mortality rate was coma caused by acute cerebrovascular disease. Conclusion The coma patients are often in serious condition, the disease progresses faster and the mortality rate is higher. This requires the pre-hospital emergency personnel to quickly judge the patient＇s condition in a short period of time and give effective initial emergency treatment. Only by ensuring that patients with coma receive good first-aid treatment can effectively improve the prognosis of patients with coma.
