摘要
目的探讨经Quadrant通道单侧椎弓根钉内固定治疗老年腰椎间盘突出症的疗效。方法选取2015年3月至2016年10月渭南市中心医院和西安交通大学第一附属医院收治的老年腰椎间盘突出症患者74例为研究对象。采用随机数表法将入选患者分为研究组和对照组,每组各37例。研究组患者行Quadrant通道下单侧椎弓根钉内固定,对照组患者行传统开放性手术。比较两组患者的手术情况(手术时间、术中出血量、手术切口长度、术后并发症)、临床疗效、视觉模拟评分法(visual analogue scale,VAS)评分及Oswestry功能障碍指数(Oswestry disability index,ODI)评分。结果两组患者手术时间和临床治疗总有效率比较均无显著差异(P_均>0.05),研究组患者术中出血量显著少于对照组(P<0.05),手术切口长度显著短于对照组(P<0.05),术后并发症发生率显著低于对照组(P<0.05)。两组患者术前VAS和ODI评分比较均无显著差异(P_均>0.05);术后两组患者VAS和ODI评分均显著低于术前(P_均<0.05),且研究组患者上述评分均显著低于对照组(P_均<0.05)。结论经Quadrant通道单侧椎弓根钉内固定手术创伤小、术后恢复快,对老年腰椎间盘突出症疗效肯定,是一种安全、可靠、有效的治疗方法。
Objective To investigate the clinical effect of unilateral pedicle screw fixation via Quadrant channel for the treatment of elderly patients with lumbar disc herniation. Method From March 2015 to October 2016, 74 cases of elderly patients with lumbar disc herniation in our hospital were selected as the subjects, they were divided into research group and control group according to the random number table method, 37 cases in each group. Research group patients were treated with unilateral pedicle screw fixation via Quadrant channel, control group patients were treated with traditional open surgery. The surgery condition（operation time, amount of bleeding, length of incision, postoperative complications）, clinical efficacy, visual analogue score（VAS） and Oswestry dysfunction index（ODI） were compared between the two groups. Result There were no significant differences in operation time and total effectiveness of clinical treatment between the two groups（Pall〈0.05）. The amount of bleeding in research group was significantly less than control group（P〈0.05）, the length of incision was significantly shorter than control group（P〈0.05）, and the incidence of postoperative complications was significantly lower than control group（P〈0.05）. There were no significant differences in the VAS and ODI scores between the two groups before surgery（Pall〈0.05）, and the VAS and ODI scores in the two groups after surgery were significantly lower than those before surgery（Pall〈0.05）, and both scores of study group were significantly lower than those in control group（Pall〈0.05）. Conclusion Unilateral pedicle screw fixation via Quadrant channel has less trauma and faster postoperative recovery, and it is a safe, reliable and effective treatment for the elderly patients with lumbar disc herniation.
