摘要
目的探究保留迷走神经和幽门微创手术在早期原发性胃癌患者中的应用效果。方法选取初诊的42例原发性EGC患者作为研究对象,患者均采取保留迷走神经和幽门微创胃切除手术,观察患者手术时间、术后出血量,观察患者术后住院时间、术后肠道首次排气时间、首次进流质食物时间、首次进半流质食物时间、术后并发症以及等指标。对术后患者随访一年,观察患者体重、食欲、腹泻、食管黏膜改变、胃部饱胀感、胆囊收缩功能等指标。结果 42例患者平均手术时间为(235.1±42.2)min,平均术中出血量为(106.2±64.8)ml。42例患者平均术后住院时间为(15.2±6.2)d,平均术后首次排气时间为(3.3±1.5)d,平均术后首次进流质食物时间为(3.7±2.1)d,平均术后首次进半流质食物时间(11.2±7.8)d。42例患者术后并发症发生率为7%,1年生存率为95%。结论采用保留迷走神经以及幽门胸腔镜微创胃切除术治疗EGC治疗效果不低于传统胃切除手术,且安全性和可行性较高,值得在胃癌治疗手术中积推广和应用。
Objective To investigate the effect of preserving vagal and pyloric minimally invasive surgery in patients with early stage gastric cancer. Methods Forty-two patients with primary EGC were enrolled in this study. All the patients were treated with preserved vagus nerve and pyloric minimally invasive gastrectomy. The operation time,postoperative bleeding volume,postoperative hospital stay,postoperative intestinal tract The first time of the exhaust,the first into the liquid food time,the first time into the semi-liquid food,postoperative complications and other indicators. Patients were followed up for one year to observe the body weight,appetite,diarrhea,esophageal mucosa changes,stomach fullness,gallbladder contraction and other indicators. Results The average operation time of the 42 patients was（235. 1 ± 42. 2） min and the average blood loss was（106. 2 ± 64. 8）ml. The average postoperative hospital stay of 42 patients was（15. 2 ± 6. 2） days,the average time to first deflation was（3. 3 ±1. 5） days,and the mean postoperative first intake of liquid food was（3. 7 ± 2. 1） days. The mean postoperative For the first time into the semi-liquid food time（ 11. 2 ± 7. 8） d. 42 patients with postoperative complications was 7%,1-year survival rate was95%. Conclusion The preservation of the vagus nerve and pyloric thoracoscope minimally invasive gastrectomy for the treatment of EGC is not less than the traditional gastrectomy surgery,and high safety and feasibility,it is worth in the treatment of gastric cancer and product promotion and application.
