摘要
目的探讨定量动态增强MRI对骨肿瘤良恶性的鉴别诊断价值。方法选取2015年2月—2017年10月我院骨肿瘤患者63例,均行定量动态增强MR I,通过计算容量转运常数、速率常数、血管外细胞外间隙容积分数等定量参数指标,计算工作特征曲线下面积;通过2组的鉴别切点,计算2组各项指标的灵敏度、特异度、准确性、阳性预测值、阴性预测值。结果容量转运常数、速率常数、血管外细胞外间隙容积分数的曲线下面积分别为0.804,0.803,0.688,最佳诊断切点为0.568,2.458,14.12。将三个最佳诊断切点作为标准,计算容量转运常数、速率常数、血管外细胞外间隙容积分数灵敏度分别为92.6%,74.0%,82.0%;特异度分别为58.3%,71.6%,52.0%;准确率分别为80.4%,73.5%,70.1%;阳性预测值分别为81.3%,84.3%,76.4%;阴性预测值分别为80.2%,62.0%,58.1%。结论定量动态增强MRI用于对骨肿瘤的良恶性诊断具有较高的参考价值,容量转运常数、速率常数、血管外细胞外间隙容积分数可作为鉴别指标,值得临床推广应用。
Objective To investigate the effect and value of quantitative dynamic enhanced MRI in the differential diagnosis of benign and malignant bone tumors. Methods 63 patients with bone tumors from February 2015 to October 2017 were selected,capacity transport constant,rate constant and extravascular extracellular space volume fraction, area under the working characteristic curve were calculated,by differential cut-off points,sensitivity,specificity,accuracy,positive predictive,egative predictive value were calculated. Results The volume of volume transport constant,rate constant and extravascular extracellular space volume fraction were 0.804,0.803,and 0.688.best diagnosis points were 0.568,2.458 and 14.12.three best cut-off points were used as the standard.The sensitivity of volume transfer constant,rate constant and extravascular volume fraction were 92.6%,74% and 82%,specificity was58.3%,71.6%,52%,accuracy rate was 80.4%,73.5% and 70.1%,positive predictive values were 81.3%,84.3%,76.4%,negative predictive values were 80.2%,62% and 58.1%.Conclusion Quantitative dynamic contrast-enhanced MRI has a reference value for the diagnosis of benign and malignant bone tumors. Volumetric transport constant,rate constant and extravascular extracellular space volume fraction can be used as a reference index,which is worthy of clinical application.
