阵发性心房颤动的散点图特点分析 被引量:4

摘要 目的探讨并分析阵发性心房颤动的心电散点图特征,并进行分类总结。方法选取2015年1月~2017年11月在我院就诊的阵发性房颤患者103例,持续性房颤患者98例,房性早搏患者121例为研究对象,通过24h动态心电图资料制成心电散点图,分析阵发性房颤、持续性房颤与房性早搏散点图之间B线斜率的差异,观察阵发性心房颤动的心电散点图特征。结果阵发性房颤心电散点图B线斜率与持续性房颤B线斜率之间差异无统计学意义(P>0.05),但阵发性房颤散点图B线斜率与房性早搏B线斜率比较有统计学差异(P<0.05)。阵发性房颤心电散点图分布特征为扇形,窦性心律分布形状为棒球拍形状,其中23例患者扇形与棒球拍形状完全分离,占比22.33%;48例患者不完全分离,占比为46.60%;30例患者完全重叠,占比为29.13%。结论阵发性心房颤动患者的心电散点图具有独特的图形特征,利用心电散点图可以对阵发性心房颤动进行快速诊断和鉴别诊断,避免漏诊和误诊,对提高医师的诊断速度和临床工作效率具有重要意义,值得临床推广应用。 Objective To investigate the characteristics of paroxysmal atrial fibrillation on RR-Lorenz plot.Methods 103 cases of paroxysmal atrial fibrillation,98 cases of persistent atrial fibrillation,121 cases of atrial premature beats who were treated in our hospital from January 2015 to November 2017 were enrolled in this study.Their 24 hour ambulatory electrocardiogram data were selected and made into RR-Lorenz plot.We analyzed the features of their RR-Lorenz plot and the difference in the slope of line B. Results The average B-line slope of paroxysmal atrial fibrillation was 0.264±0.062.There was no significant difference between the B-line slope and the B-line slope of persistent AF(P〉0.05),but paroxysmal The B-line slope of atrial fibrillation scatter plot was significantly different from the B-line slope of atrial premature beat(P〈0.05).The features of paroxysmal atrial fibrillation and sinus rhythm on RR-Lorenz plot was fan-shaped and baseball bat shape individually.Among them,fan-shaped and baseball bat shape completely separated in 23 cases,accounting for 22.33%.48 cases were not completely separated(46.60%)and 30 cases were completely overlapped(29.13%). Conclusion Patients with paroxysmal atrial fibrillation has a unique graphical features on RR-Lorenz plotthat is helpful for clinical diagnosis of paroxysmal atrial fibrillation,to avoid missed diagnosis and misdiagnosis and improve the clinician efficiency.It is worthy of clinical application.
