摘要
胰腺癌发病率在全球范围内呈明显上升趋势。目前胰十二指肠切除术(PD)手术切除仍是唯一可能治愈胰腺癌的治疗手段。为尽可能全面的淋巴结清扫、提高PD术后R0切除率、提高胰头癌远期疗效、减少复发。国内外学者在手术切除的范围、切除的方法及手术的入路等方面,做出了许多探索。以肠系膜上动脉(SMA)为入路PD手术可在术中对SMA及腹腔干进行全面探查,真实可靠的判断局部血管受侵犯程度及肿瘤的可切除性,提高手术切除率,改善预后。现临床上常用的SMA入路的PD的方法主要有6种。我们将近几年来关于SMA入路PD的手术方法、相关临床应用及研究进展进行综述。

The superior mesenteric artery as the ap- proach of the PD surgery in the superior mesenteric artery and abdominal cavity to conduct a comprehensive exploration, a reliable and reliable assessment of local vascular invasion and tumor resectability, improve surgical resection, improve the prognosis. The PD with superior mesenteric artery approach could complete a comprehensive exploration on the mesenteric artery and abdominal cavity in surgery, determine the degree of local vascular invasion and tumor resectability reliably, improve the surgical resection rate and improve the prognosis. There are six main methods of clinical use of the superior mesenteric artery(SMA) approach. The author summarizes the surgical methods, related clinical application and research progress of SMA approach in PD in recent years.
