摘要
目的比较不同置入途径下使用Y形钢板和双钢板治疗肱骨髁间骨折的疗效。方法以2013年2月至2016年2月70例肱骨髁间骨折手术治疗患者为研究对象,依据手术所选择的固定材料将所有患者分成两组:双钢板组(35例)和Y形钢板组(35例)。两组患者中均各有18例采用尺骨鹰嘴截骨置入途径,17例采用肱三头肌舌形肌瓣置入途径。比较两组患者的治疗总有效率,关节功能以及生活能力恢复情况,疼痛以及负面情绪发生情况。结果两组患者治疗总有效率、关节功能以及生活能力恢复情况、疼痛以及负面情绪发生情况均无差异(P>0.05)。采用尺骨鹰嘴截骨置入途径的患者,其关节功能及生活能力恢复情况、术后疼痛以及负面情绪控制均优于同组采用肱三头肌舌形肌瓣置入途径的患者(P<0.05)。结论使用Y形钢板和双钢板固定材料治疗肱骨髁间骨折均有较好的治疗效果,其中采用尺骨鹰嘴截骨置入途径的手术方式更为有效。
Objective To compare the efficacy of Y-shaped plate and double plate in the treatment of humeral intercondylar fractures through different approaches.Methods There were 70 patients with humeral intercondylar fractures who were admitted to our hospital from February 2013 to February 2016.All these patients were divided into the double plate group or the Y-shaped group according to the fixation methods.Each group had 35 cases,of which 18 cases underwent olecranon osteotomy and 17 cases were fixed through the tongue-shaped flap.Comparison of the total efficacy of treatment,joint function and life capacity recovery,pain and negative emotions was performed between two groups.Results There were no significant difference in the total efficacy rate,joint function and recovery of life ability,pain and negative emotions between the Y-shaped plate group and the double-plate group（P〉0.05）.Nonetheless,the olecranon osteotomy group were superior to the tongue-shaped flap group in the joint function and recovery of life ability,pain and negative emotions,and the difference was statistically significant（P〈0.05）.Conclusion Both the Y-shaped plate and double plate fixation methods for the treatment of humeral condylar fractures could achieve satisfactory results;the fixation via an olecranon osteotomy approach would yield better results.
