摘要
目的了解慢性心力衰竭(chronic heart failure,CHF)患者对指南推荐药物的应用情况。方法回顾性分析406例CHF患者的药物使用率。结果心功能Ⅱ级:利尿剂类药物使用率为69.8%,血管紧张素转换酶抑制剂(ACEI)或血管紧张素Ⅱ受体拮抗剂(ARB)类药物使用率为55.2%,β受体阻断剂使用率为57.2%,强心苷类药物使用率为51.0%,醛固酮受体拮抗剂类药物使用率为60.4%,硝酸酯类药物的使用率为19.8%;心功能Ⅲ级:利尿剂类药物使用率为78.8%,ACEI或ARB类药物的使用率为54.2%,β受体阻断剂的使用率为56.1%,强心苷类药物的使用率为52.8%,醛固酮受体拮抗剂类药物的使用率为72.2%;心功能Ⅳ级:利尿剂类药物的使用率为84.7%,ACEI或ARB类药物的使用率为68.3%,β受体阻断剂的使用率为54.1%,强心苷类药物的使用率为64.3%,醛固酮受体拮抗剂类药物的使用率为78.6%,硝酸酯类药物的使用率为27.6%。结论利尿剂、强心苷类、ACEI或ARB、β受体阻断剂类药物的使用率不理想,醛固酮受体拮抗剂主要用药为螺内酯。
Objective Through a retrospective analysis of chronic heart failure patients with drug treatment,to understand the application guidelines recommend drugs. Methods A retrospective analysis was used to record the drug treatment of 406 cases of chronic heart failure. Results For patients with cardiac function II,the utilization rate of diuretic drugs（ including spironolactone） was 69. 8%,the ACEI/ARB was 55. 2%,the beta blocker was 57. 2%,the digitalis was 51%,the aldosterone receptor antagonist was 60. 4%,the nitrates was 19. 8%. For patients with cardiac function III,the utilization of diuretic drugs（ including spironolactone） was 78. 8%,the ACEI/ARB was 54. 2%,the beta blockers was 56. 1%,the digitalis was 52. 8%,the aldosterone receptor antagonists was 72. 2%. For patients with cardiac function IV,the utilization of diuretic drugs（ including spironolactone） was 84. 7%,the ACEI/ARB was 68. 3%,the beta blocker was 54. 1%,the digitalis was 64. 3%,the aldosterone receptor antagonist was 78. 6%,the nitrates was 27. 6%. Conclusion The utilization ratio of diuretics,cardiac glycoside,ACEI/ARB and Bata blockers is not ideal. The main drug of aldosterone receptor antagonists is spriono lactone.
