摘要
非酒精性脂肪胰腺疾病合并糖耐量低减与中焦(脾胃)失运关系密切,基本病机为气阴两虚,并因"阴虚血滞""气虚浊留",即:气虚——脾不散精,阴虚——津伤内热,血瘀——消因瘀成、瘀因消重,故此时已有络脉瘀阻之征。故治疗上应采用益气养阴活血组药,基本原则及组药包括:(1)益气健脾以复运化,黄芪、生地黄、灵芝合用益气养阴、补运结合、恢复脾运;(2)辛润养阴以醒脾,苍术、女贞子、决明子合用滋阴生津、燥湿运脾;(3)活血通络以防变,葛根、丹参、制大黄合用养阴生津以滋血、活血祛瘀以生新。
Non-alcoholic fat pancreatic diseases with impaired glucose tolerance have a close relationship with the dysfunction of Middle-burner(spleen and stomach）. The basic pathogenesis is deficiency of both Qi and Yin. Then the conditions of ＂ blood stagnation due to Yin deficiency＂ and ＂ turbidity retard due to Qi deficiency＂ come into being. In other words,Qi deficiency leads to the problem of Spleen being not able to scatter Essence and Yin deficiency leads to body fluid injury and heat stagnation,which in turn causes blood stasis. Therefore,at this time there is a new problem of collateral blood stasis and resistance. Therefore,in treatment,group drugs which can tonify Qi and Yin as well as activating blood circulation should be used. The basic principles of the using of the group drugs include the followings：（1）Tonifying Qi and Spleen to recover the normal transformation. Drugs like Astragalus,Rehmanniae radix,Ganoderma lucidum can be used in combination to tonify Qi and Yin and eventually achieve the goal;（2）Drugs with Xin and Run in flavours should be used to tonify Yin and to strengthen the power of the Spleen. Drugs like Cangshu,Ligustrum lucidum and Cassia seed can be used in combination to nourish Yin and body fluid as well as dispeling Dampness to tonify the Spleen;（3） Activating blood circulation and drgdging collaterals to prevent further injury with body. Drugs like Pueraria,Salvia and Rhubarb can be used in combination to tonify Yin,increase body fluid and blood,and to remove blood stasis to improve the condition.
